Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) document subpart 12.6, as supplemented with additional information included in this notice. This Announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation number F23606 00 Q6005 is being issued as a RFQ. The solicitation is 100% small business set-aside and size standard is $5.0M. The Standard Industrial Code (SIC) is 4952. The Defense Priorities and Allocations System (DPAS) rating is S-10. Evaluation/Award: The Government will award a contract resulting from this solicitation to the responsible offeror providing the lowest overall price. Blanket Wage Determination No. 94-2307, Revision No. 16, dated 06/04/2000, shall be incorporated in the resulting award. This statement is for information only not a wage determination: Employee Class 23930, Telecommunications Mechanic I, WG-10, Monetary Wage-Fringe Benefits $20.35. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-05 and Defense Acquisition through Federal Acquisition Circular (FAC) 97-05 and Defense Acquisition Circular (DAC) 91-13. The following FAR Clauses and Provisions apply to this acquisition: Provision 52.204-6, Contractor Identification Number-Data Universal Numbering System (DUNS) Number; Provision 52.212-1, Instructions to Offerors Commercial Items; Provision 52.212-3, Offeror Representations and Certifications Commercial Items; Clause 52.212-4, Contracts Terms and Conditions Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items; the following subparagraphs apply: (a), (b)(3), (b)(5), (b)(6-11), (c)(1-2), and (e); DFARS Provision 252.204-7004, Required Central Contractor Registration (Mar 1998); DFARS Clause 252.232-7009, Payment by Electronic Funds Transfer (CCR) (JUN 1998). Registration is required in the CCR database in order to receive a Government contract. DFARS Provision 252.212-7000, Offeror Representations and Certifications Commercial Items and DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. Offerors shall include a completed copy of FAR Provision 52.212-3 and DFARS Provision 252.212-7000 with their quote. The above Provisions and Clauses may be obtained via Internet http://farsite.hill.af.mil. Description: Furnish all labor, materials and equipment to provide local and nation-wide cellular telephone service, for 128 Government owned cellular telephones, for Whiteman Air Force Base, Missouri. To include a standard 10-digit telephone number (including area code and 7-digit telephone number), compatible with nation-wide dialing plans. The local coverage includes all toll free counties within the State of Missouri. The nation-wide coverage includes the 48 contiguous states within the U.S., to include the District of Columbia. This service shall provide the user with traveling capability without losing incoming calls from their local area. Monthly payment authorized. Response time: The Government may place service calls 24 hours a day, 7 days a week (excluding Federal Holidays). The contract manager or alternate shall be available during normal duty hours within two (2) hours to meet on the installation with government personnel (designated by the Contracting Officer (CO)) to discuss problem areas. After normal duty hours, the manager or alternate shall be available within three (3) hours. Pick-up and Delivery: The contractor shall pick up and deliver cellular telephones at the 509th Communications Squadron, 920 12th Street, Whiteman AFB MO, Land Mobile Radio shop, hours of operation are Monday-Friday from 7:30 A.M. to 4:30 P.M. Reprogramming Cellular Phones: The contractor shall reprogram government furnished cellular telephones at the beginning of the contract, and when required during the contract performance period. The contractor shall begin reprogramming all cellular telephones within 1 to 3 days after award of the contract. The contractor and the government will determine a work schedule to reprogram the cellular telephones. The government will incur no activation fee for cellular telephones activated at the start of and throughout this contract. Electronic Serial Number Changes (ESN): The Governmennt will incur no cost for ESN changes of cellular telephones. Billing Statements: The contractor will provide itemized (per telephone), detailed monthly billing statements for each telephone covered. Upon request, the contractor without additional cost to the government will provide an itemized telephone bill. Billing statements will be sent no sooner than the first day of the month after the service was received, prebilling is not authorized. Billing statements will be structured in a fleet/subfleet format as designated by the government. The contractor will provide a minimum of two copies of each billing statement to 509CS/SCXB, 920 12th Street, Whiteman AFB Mo 65305. Local Dialing Access: The contractor will provide, free of charge, a means to make calls from the 687 exchange to a government owned cellular telephone with no long distance toll charge being incurred. This requirement may be met through normal landline operation of electrical (transceiver) operation. A minimum of 10 outgoing lines must be provided for this function. Cellular Telephone Charges: Telephones, which are not turned on during any given month, will not be charged a monthly charge. Once the telephone is activated during any given month, then the monthly surcharges, as well as access fees, may be accessed to the telephone. Additional telephones may be added to the contract without additional activation charges. Telephones may be removed without early cancellation penalties/charges to the government. Service to be for the period of 1 October 2000-30 September 2001. Offers shall be received at the address shown above, or if hand carried, placed in the depository located at 727 Second St., Suite 124A, Whiteman AFB, MO no later than 26 September 2000, 4:00 P.M. local time.
