Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. II. This solicitation MDA220-00-Q-0035 is issued as an RFQ. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 19. IV. N/A. V. A list of contract line item number(s) is as follows: CLIN 0001, DESCRIPTION -- Memberships base year for the period beginning date of contract award through 30 Sep 2001, QTY -- 125; CLIN 0002 (Option Year 1), DESCRIPTION -- Memberships for the period 01 October 2001 through 30 September 2002, QTY -- 125; CLIN 0003 (Option Year 2), DESCRIPTION -- Memberships for the period 01 October 2002 through 30 September 2003, QTY -- 125; CLIN 0003 (Option Year 3), DESCRIPTION -- Memberships for the period 01 October 2003 through 30 September 2004, QTY -- 125; CLIN 0004 (Option Year 4), DESCRIPTIONS -- Memberships for the period 01 October 2004 through 30 September 2005, QTY -- 125. VI. The Contractor shall provide unlimited access to the facility, qualified instructors, formal health programs and services. The facility shall be easily accessible by employees before and after duty hours and during the normal business day. The facility shall be within reasonable walking distance of Crystal Mall 3, 1931 Jefferson Davis Highway, Arlington, VA. The facility shall provide a variety of exercise equipment and classes to include aerobic exercise machines, weight room, pool, aerobics classes, tennis courts, racquetball courts, and shower facilities. The Contractor shall provide numbered, individual picture identification cards to members at no additional cost to the government. The Contractor must maintain a listing of all cards issued each month and must also maintain a daily sign-in-log and prepare reports (i.e. weekly maintenance log/report, monthly fitness center report, etc.). The Contractor shall ensure that the utilization of memberships does not exceed the estimated number listed in section b, without prior approval from the Contracting Officer. This Membership shall belong to the Government (DFAS). The Contractor shall propose a mechanism for structuring and pricing a corporate membership or memberships for DFAS. The Government (DFAS) shall have the right to transfer memberships from person to person at will, provided that the individual assuming the membership makes the requisite co-payment. There shall be no fee associated with such a transfer. In the event that a DFAS member transfers or decides not to continue using the facility during the period of this contract, the Contractor shall propose a mechanism for structuring and pricing of the remaining balance. The Government (DFAS) will be allowed to hold Performance Readiness Test (PRT) testing twice per year at no cost to DFAS military personnel who are not members of the facility. The PRT testing time/date must be approved prior to test. VIII. FOB Destination. VIII. FAR 52.212-2 carried the following addendum: The following factors shall be used to evaluate offers: (1) past performance, (2) location, (3) price. Technical factors are more important than price. Award shall be made on a best value basis. Contractor must be registered in the Central Contractor Registration (CCR) in order to be eligible for contract award. X. Offerors shall include a completed copy of the provision at FAR 52.212-3 Offerors-Representatives and Certification-Commercial Items. XI. FAR 52.212-4 Contract Terms and Conditions-Commercial Items is incorporated by reference and applies to this acquisition and resulting contract. XII. FAR 52.212-5 Contract terms and Conditions, Required To Implement Statues Or Executive orders-Commercial Items is incorporated by reference. XIII.
