- Description
- This is a combined synopsis/solicitation for commercial items/services prepared in accordance with format in sub parts 12.6 and 13.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued, proposals are being requested from this announcement, a written solicitation will not be issued. The Request for Proposal number is 575-529-01. This solicitation incorporated provisions and clauses in effect through FAC 97-19. Standard Industrial Code of 8999 is applied. This solicitation is issued subject to the availability of government funds at time of award. This solicitation is unrestricted for the following service: Contractor shall provide maintenance services for the following Toshiba Ultrasound equipment: SSH-140A Ultrasound System, PEF-510SB Transducer Transesophagael, PEF-704LA Laparascopic Transducer, PSF-37FT Sector Transducer, PLF-703NT Linear Transducer, PVF-620ST Monoplane Transducer, PSF-25LT Sector Transducer, PSF37HT Sector Transducer, SVO-9500MD/4 S-VHS Sony VCR, UP-890MD B&W Sony Printer and SSA-270A Ultrasound System, PLF-703ST Linear Transducer, PSF-25DT Sector Transducer, PSF-37CT Sector Transducer, PVF-357MT Convex Transducer, PVF-745V Intra operative Transducer, UP-3000 Sony Color Printer, AG-7350 VCR, S-VHS Panasonic. Maintenance is to include two (2) preventive maintenance visits per year, all parts and labor during business hours (8:30-5 Monday through Friday). The following provisions and clauses are applicable and are hereby incoporated into the solicitation by reference and will be provided by the contracting officer in full text upon request: FAR 52.212-1, Instruction to Offerors Commercial Items applies. Offerors are Required to complete and include a copy of the following provisions with their proposals: FAR 52.212-3, Offeror Representations and Certifications Commercial Items, FAR 52.212-5 Affirmative Action for Special Disabled and Vietnam Era Veterans: FAR 52.222-36 Affirmative Action for Handicapped Workers; FAr 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era. Award will be made to that offeror (1) whose proposal is technically acceptable and (2) whose technical/cost relationship is most advantageous to the Government. While cost is secondary to technical, it will be a factor in the award decision. Also, award may not necessarily be made for technical capabilities that would appear to exceed those needed for the successful performance of the work.
