RFQ# F65503-01QR041 -- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. **NOTE: In order to receive a government award, you must be registered with the Central Contractor Registry. You can obtain information on registering with CCR at their web site www.ccr2000.com** This acquisition is set-aside and reserved exclusively for small business concerns. If no acceptable offers are received from responsible small business concerns, the set-aside shall be withdrawn and the requirement, if still valid, shall be re-solicited on an unrestricted basis. (Reference FAR Part 19) The SIC code that applies is 2899, 500 employees for vendor and affiliates is the maximum number to still be considered small. The applicable clauses, invoicing procedures and representations and certifications are attached as separate files. Line Item 0001: Quantity 4750 ea. Sodium Chloride_packaged & shipped in bags with individual net weights not less than 40 pounds & not more than 60 pounds; individual bags shall contain no less than 98 percent of the stated net weight. Contractor will make two deliveries_one half delivered within 30 days of contract award, second half delivered NLT 15 May 2001. Price must include delivery **Line Item 0002: Quantity 26 ea. Aqua Mag_a proprietary liquid ortho/polyphosphate blend manufactured by Carus Chemical Company; packaged & shipped in plastic drums with individual net capacity of 30 gallons. Contractor will deliver full amount within 30 days of contract award.**Line Item 0003; Quantity 30 ea. Sodium Fluoride_dry, coarse crystalline grade; sodium fluoride content shall equal at least 97 percent (dry basis); product must comply with ANSI/AWWA standard B701-94 or most current revision; packaged & shipped in bags with individual net weight equal to 50 pounds; individual bags shall contain no less than 98 percent of the stated weight.
