Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number N00168-00-Q-1152 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. The Acquisition Management Department, National Naval Medical Center has a requirement for the following items: CLIN 0001: One (1)each 19350II-CB-M Moria Carriazo-Barraquer Manual System with Evolution II Console which includes one (1)each l9350 Evolution II Carriazo-Barraquer Control Unit (with dual vacuum level, LED, dual suction level, smart audible); one (1)each 19351 Locking Footswitch; one (1)each 19152/110 Power Supply; one (1)each 19303 Manual Turbine; two (2)each 19117 Moist Chamber; one (1)each 19353 Turbine Flexible Hose; one (1)each Shipping Case; one (1)each l9108, 19309 Lubricant Spray and Tip; two (2)each 19343 Sterile Tray Insert; two (2) each 19139 Sterile Tray (C-B head, fixation rings, turbine motor, and turbine flexible hose); two (2) each 19135 Blade Extractor; two (2) each 19307/130 C-B Microkeratome Head; two (2) sets 19309/-1.0,+1,+2 Fixation Ring, Manual; four (4)each 19310 x 2, 19311 x 2 Applanator Lens; one (1)each 19128 Cleanpack Protection; one (1)each 19120 Nitrogen Hose to tank; one (1)box 19138 Sterile Tubing; one (1)box 19315 Sterile C-B Blades; CLIN 0002: One (1)each OBT-65 Tonometer; CLIN 0003: One (1)each Microkeratome cart with pull out mayo stand with nitrogen tank enclosure. Two (2) complete sets of operators, maintenance and training manuals shall be supplied for the equipment. Equipment should be delivered F.O.B. Destination, Material Officer, Bldg. 54, National Naval Medical Center, Bethesda, MD. FAR 52.212-1 "Instructions to Offerors-Commercial", is incorporated by reference and applies to this acquisition. FAR 52.212-2 "Evaluation-Commercial Items" applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: technical capability and price. Technical capability is approximately equal to price. Award will be made to the offeror that meets the solicitation's minimum criteria for technical acceptability at the lowest price. To be determined technically acceptable, the offeror must furnish product literature that demonstrates the products offered meets all requirements stated in the above purchase description. Offerors shall include a complete copy of the provision FAR 52.212-3, "Offeror Representations and Certifications -- Commercial Items", with its proposal. FAR 52.212-4 "Contract Terms and Conditions -- Commercial Items" is incorporated by reference and applies to this acquisition and resulting contract. FAR 52.212-5 "Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items (Aug 1996) is incorporated in this contract by reference, however for paragraphs (b) and (c) the following provisions apply to this acquisition: FAR 52.222-26, Equal Opportunity (E.O. 11246), FAR 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans (38 U.S.C. 4212), FAR 52.222-36, Affirmative Action for Handicapped Workers (29 U.S.C. 793), FAR 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era (38 U.S.C. 4212), FAR 52.225-3 Buy American Act -- Supplies (41 U.S.C. 10).
