Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information contained in this notice. This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the only solicitation a written solicitation WILL NOT be issued. The solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-19. The VA intends to award a firm-fix priced contract for Computerized Clinical Information System (CCIS). WJBD Veterans Affairs Medical Center (VAMC) plans to negotiate on a sole source basis with Micromedix, Inc, 6200 S. Syracuse Way, Suite 300, Englewood, CO. The Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capabilities to respond to the requirement or submit proposals. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. A DETERMINATION BY THE GOVERNMENT NOT TO COMPETE THIS CONTRACT BASED UPON RESPONSES TO THIS NOTICE IS SOLELY THE DISCRETION OF THE GOVERNMENT. INFORMATION RECEIVED WILL BE CONSIDERED SOLELY FOR THE PURPOSE OF DETERMINING WHETHER TO CONDUCT A COMPETITIVE PROCUREMENT. The NAICS Code is 42143 and the size standard is 100 employees. The contractor shall provide Intranet Version Bundled Database selection for all VISN 7 facilities, clinics and centers, for the period October 1, 2000 through September 30, 2001, to include individualized customer service and support, including Technical Professionals 24 hours a day, Editorial Specialists, and Customer Support Representative. Updates are to be provided every 90 days, in accordance with all terms, conditions and provisions. The following Intranet version bundled databases are: (1) POISINDEX System; (2) DRUGDEX System; (3) EMERGINDEX System; (4) IDENTIDEX System; (5) TOMES System; (6) CARENOTES, (7) DRUG-REAX System; (8) REPRORISK System; (9) MARTINDALE; (10) Material Safety Data Sheets (MSDS); (11) PDR; (12) AltMedDex; and (13) RPS Herbal Medicine. The Contractor shall deliver all items FOB Origin to : Pamela (Jan) Burns, Chairperson, VISN-7 Library Council, VAMC Medical Center (14A), 700 South 19th Street, Birmingham, AL 35233. The contractor shall submit a price for each item on company letterhead and include a completed copy of Provision 52.212-3 Representations and Certification Commercial Items and 852.219-70 Veterans-Owned Small Business and confirmation of completions of VETS-100 Report, to the Contracting Officer at the address indicated above, by close of business 9/25/00.
