Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in [FAR] Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number RFQ 672-142-00 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-19. Solicitation number RFQ672-142-00 Request for Quote is 100% set-aside for small business under SIC code 2599 with a business size of 500 employees. The Government contemplates awarding a Firm-Fixed Price. The requirement is for Positron Emission Tomography (PET) furniture for Nuclear Medicine Service "hot lab" and includes installation on site. Equipment is lead-shielded, storage modules with key locks, specially designed for areas where radioactive materials such as radiopharmaceuticals, radionuclides, and small sources are handled and stored. The modules are lead-shielded for radiation protection and the interior and exterior metal surfaces are bake-painted for corrosion resistance and easy decontamination and cleaning. VAMC will remove existing modules; existing electrical outlets in place will be used. OFFEROR SHOULD REQUEST DIAGRAM OF THE LAYOUT BEFORE SUBMITTING QUOTATION. OFFEROR SHALL SUBMIT A DIAGRAM OF THE LAYOUT FOR THE NEW HOT LAB MODULES WITH THE QUOTATION. CLIN 0001 Preparation and Fume hood Enclosure. Counter-mounted fume hood supported by enclosure base module. Enclosure is completely shielded with " lead and encased in a stainless steel jacket. gaseous radiopharmaceuticals emitted through rear baffle with exhaust slot and adjustable lower exhaust slot. Stainless steel chimney connects to external ductwork. Feature: sliding shield with leaded shielding 1 Each $__________. Left side provides stainless steel sink with gooseneck faucet, anti-splash nozzle, drain and overflow system. Compartment may be used for cold storage only. Right side provides two chutes that allow waste to be dropped directly into stainless steel waste trays. Five trays for short half-life material, and three for long half-life material. CLIN 0005 Generator and Storage Module Provides radiation protection without hampering the elution process. Drawer for active generator use and can accommodate either top or front-elution generators. Work will consist of furnishing all plans, labor, materials, tools, equipment, furniture (modules), removal of all packaging material outside of VA premises in accordance with Federal and local codes, and transportation necessary to perform all operations in connection with the installation of described furniture. All equipment to be delivered at same time. No partial delivery, Work should begin within 5 working days of the delivery of all equipment..
