Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes a formal Request for Quotation (RFQ 500-41-00) and a written solicitation will not be issued. It incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 97-19. The Standard Industrial Classification (SIC) Code for this acquisition is 8999. Prices should be submitted for the following services: a) Parts and labor necessary to perform preventive maintenance on (9) nine Puritan Bennett Adult Microprocessor Controlled Enhanced Ventilators, Model 7200AE. Work to be done on site. Separate quotes should be submitted for Level I and Level II preventive maintenance work. b) Parts and labor necessary to perform preventive maintenance on (1) one Infrasonics Adult Star Original Ventilator. Work to be done at vendors facility. c) Parts and labor necessary to perform preventive maintenance and install a Y2K upgrade (Ver 219.4) kit on (3)three Infrasonics Adult Star Ventilator, Model 2000. Work to be done at vendors facility. VA to supply Y2k Upgrade kit. d) This solicitation should also include the hourly labor rate for repairs that may be necessary above and beyond the preventive maintenance work. VA Biomedical employees will coordinate delivery of ventilators. Offers shall be submitted FOB Destination. The following factors, in descending order of importance, will be used to evaluate quotes: (1) lowest price quotations, (2) repair schedule, and (3) technical ability.
