Description: IMPORTANT NOTICE: DFARS 252.204-7004 -- Required Central Contractor Registration (CCR) applies to all solicitations issued on/after 6/1/98. Call 1-888-227-2423 or view at http://www.ccr2000.com for more information. Solicitation number N00421-00-Q-0644 is issued as a Request for Quote (RFQ). This is a notice to negotiate with Life Fitness, 10601 West Belmont Avenue, Franklin Park, IL. Life Fitness is the original equipment manufacturer and owns the proprietary data rights to the equipment. This acquisition is being pursued a sole source basis to acquire annual maintenance and repair of fitness equipment and maintain the following equipment; 8 each Life Cycle (LC) LC 9500R HR, 10 each LC 9500 HR, 3 each Lift Step LS 9500 HR, 8 each Cross Trainer, CT 9500 HR, 16 each Treadmill, TR 9500 HR and 12 each Motorized strength machines Lift Circuit. The statutory authority permitting other than full and open competition is FAR 6.302-1; only one responsible source and no other supplies or services will satisfy agency requirements.
