Description
This is a combined synposis/solicitation in accordance with the format in FAR 12.6, and FAR 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The Request For Quotation (RFQ) number is 539-01-05. The performance period is from date of award though September 30, 2001, with four one-year options to renew. A firm fixed price contract will be awarded. The SIC Code for this service is 7389 and the NAICS Code is 54199. The size standard is $5 million in annual sales. This solicitation incorporates the provisions and clauses in effect through Federal Acquisition Circular 97-19. This procurement is unrestricted but is potentially reserved for HUBZone 8a, 8a, HUBZone Small Business, Very Small Business and Small Business participation. The contract will be awarded in the cascading order above in order to provide maximum practical opportunities for small and small disadvantaged businesses. All responses from responsible sources will be fully considered. In accordance with Federal Acquisition (FAR Subpart 19) any award resulting from this solicitation will be made on a competitive basis to an eligible HUBZone 8a concerns, provided that a minimum of 2 competitive offers are received form eligible HUBZone 8a concerns. If a minimum of 2 offers from eligible HUBZone 8a concerns are not received, the award will be made to eligible 8a concerns, and so on through the cascading order above. If a minimum of 2 competitive offers from qualified small businesses is not received, the RFQ will be competed on a full and open basis among responsible firms. This RFQ is being issued to provide annual inspection services on the boilers at the following 3 facilities: VA Ft. Thomas Nursing Home, Bldg. 64, 1000 S. Ft. Thomas Avenue, Ft. Thomas, KY 41075; VA Medical Center, 4100 W. Third Street, Dayton, OH 45428; and VA Medical Center, 3200 Vine Street, Cincinnati, OH 45220. The VA Ft. Thomas Nursing Home has (2) 200 horsepower Cleaver Brooks boilers. Type "A" internal inspections are done on the boilers annually in November/December. Type "C" external inspections are done during the same time period. The VA Medical Center in Dayton, OH has (4) VC type Erie City Ironworks water tube boilers. Serial numbers 96287 and 96288 have 40,000 horsepower capacity, and serial numbers 96285 and 96286 have 20,000 horsepower capacity. Type "A" internal inspections are done on each boiler during the summer. Type "C" external inspections are done during the same time period. In addition, Type "E" D.A. tank inspections are done in alternating years. There will be 3 of these inspections done in a 5-year period. The VA Medical Center, Cincinnati, OH has (3) Titusville Ironworks Class A 406 horsepower boilers. In the first year the inspections will be done in November/December. The inspections may be scheduled in the summer during the option years. Quotes must include a completed copy of the provision 52.212-3 Offeror Representations and Certifications-Commercial Items (OCT 2000), the DUNS number in accordance with 52.204-6 Contractor Identification Number-Data Universal Numbering System (JUN 1999), and VAAR 852.219-70, Veteran-Owned Small Business (DEC 1990).
