- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information provided in this notice. This announcement constitutes the only solicitation: A quote is being requested and a written solicitation will not be issued. Solicitation number F6550100QP004 is issued as a Request for Quotation (RFQ). The incorporated document provisions and clauses are those in effect through Federal Acquisition Circular 97-17. The Standard Industrial Classification Code is 8744 with a Small Business size standard of $5.0 million. Contractor shall provide all parts, labor, tools, equipment, supervision, personnel, materials and transportation necessary to fulfill the requirements of the Program Coordinator of the Family Services Program (FSP) in accordance with (IAW) the Performance Work Statement (PWS). Delivery terms shall be FOB destination with service to take place at Bldg 5188, 8th St, Elmendorf AFB, Alaska 99506. Performance period will be one (1) base year and three (3) option years. The following provisions and clauses are incorporated and shall remain in full force in any resultant purchase order; FAR Part 52.212-1; Instructions to Offerors, Commercial Items is incorporated by reference. FAR 52.212-2, Evaluation _ Commercial Items the blanks are completed as follows: (a) the award will be made based on price and price related factors, N/A. The government reserves the right to make award on the initial quote received without discussions. Each offeror shall include a completed copy of the provision at FAR 52.212-3; Offeror Representations and Certifications- Commercial Items with its quote. FAR Part 52.212-4; Contract Terms and Conditions- Commercial Items, is incorporated by reference and applies to this acquisition. Incorporate the clause by reference at 52.212-5; Contract Terms and Conditions Required to Implement Statutes or Executive Orders _ Commercial Items. However, for FAR clause 52.212-5 paragraph (b), only the following clauses apply: 52.203-6; Restrictions on Subcontractor Sales to the Government, with Alternate I, 52.219-8; Utilization of Small Business Concerns, 52.222-21; Prohibition of Segregated Facilities, 52.222-26; Equal Opportunity, 52.222-35; Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, 52.222-36; Affirmative Action for Handicapped Workers with Disabilities, 52.222-37; Employment Reports on Disabled Veterans and Veterans of the Vietnam Era,52.222-41; Service Contract Act of 1965, as Amended, 52.225-1; Buy American Act _ Balance of Payments _ Supplies, 52.232-33; Payment by Electronic Funds Transfer-Central Contractor Registration, Government Property (Fixed Price Contracts), 52.245-2; Government Property (Fixed Price Contracts), Alternate 1 (Deviation), 52.245-2, and, 52.247-64; Preference for Privately Owned U.S. _ Flag Commercial Vessels, Incorporate DFARS 252.204-7004; Required Central Contractor Registration. The following clauses are hereby added by addendum 52.217.8 Option to Extend Services and 52.217-9 Option to Extend the Term of the Contract. Department of Labor, Service Contract Act, Wage Determination no. 94-2017 Revision 21 dated 04 June 2000 is applicable. Each offeror shall include a completed copy of the provision at DFARS 252.212-7000 Offeror Representations and Certifications _ Commercial Items and with its quote; 52.228-5, Insurance_Work on a Government Installation; 52.245-4; Government Furnished Property (Short Form). Incorporate DFARS 252.212-7001; Contract Terms and Conditions Required to Implement Status or Executive Order Applicable to Defense Acquisitions of Commercial Items. However, for paragraph (b) only the following clauses apply, 252.225-7007; Buy American Act_Balance of Payments Program, 252.225-7012; Preference for Certain Domestic commodities, 252.227-7015; Technical Data_Commercial Items, 252.227-7037; Validation of Restrict Markings on Technical Data, 252.243-7002; Requests for Equitable Adjustment, and 252.247-7024; Notification of Transportation of Supplies by Sea. Acceptance of the warranty does not waive the government_s right with regard to the other terms and conditions of the contract. In the event of a conflict, the terms and conditions of the contract shall take precedence over the warranty. A firm-fixed price purchase order will be issued under the Simplified Acquisition Procedures. While on the Internet, please complete the Central Contractor Registration (CCR), as this is a requirement prior to award of any contract. Additional information on registration is available at 1-888-227-2423. DESCRIPTION OF SERVICES: 1.1 SCOPE OF WORK. TheContractor shall provide all personnel, vehicles, supervision, and other items necessary to perform the Resource Room Assistant services as defined in this performance work statement (PWS), except as specified in paragraph 4.0 as government-furnished property and services. Services will be performed at the Family Support Center (FSC), Elmendorf AFB Alaska. QUALIFICATIONS: Contractor shall have strong customer service skills and the necessary computer experience or training to perform the requirements of the PWS. 2.1 The contractor shall have knowledge of and/or experience in: 2.1.1 Customer service, to include listening to and assisting clientele with agency/program services. 2.1.2 General office procedures, such as basic filing practices and proper telephone courtesy 2.1.3 Effective communication skills. 2.1.4 Use of Microsoft (MS) Windows based computer software programs (i.e., MS Word; Access; Excell; MS Publisher), employment application forms (i.e., federal forms SF [Standard Form] 171, Optional Form 612, Resumix; local, state and municipal application forms) and job search related programs. 2.1.5 Use of internet, to include accessing various jobs search resources (i.e., State of Alaska homepage; Municipality of Anchorage; Federal Job Search; America_s Job Bank; Monster Board; Air Force Personnel Center). 2.2 Each offeror must submit with their proposal a: 2.2.1 Resume, and college transcripts when applicable, showing they meet the outlined requirements. 2.2.2 Three current letters of reference with the name, address, and current phone number of the individual writing the recommendation. 3.0 SPECIFIC DUTIES: 3.1 The contractor shall be responsible for the daily operation of FSC Transition Assistance Program (TAP) Resource Room services, to include client assistance, use of hardware and software, maintenance of job search library materials and job boards. This shall include: 3.1.1 Providing information to clients utilizing Resource Room services on walk-in, appointment, or telephonic basis. 3.1.2 Maintaining appointment schedule for use of Resource Room computer stations by FSC clientele. 3.1.3 Assisting Resource Room clients with familiarization and use of installed computer software programs (i.e., federal Jobs Kit, Alaska Career Information System, Salary Assessor, Resume Maker), monitoring computer use to preclude inappropriate use as prescribed by U.S. Air Force policy. 3.1.4 Collecting, reviewing, and posting job announcements received daily from employers, keeping job boards current. Ensure TAP and Career Focus Program (CFP) staff review announcements prior to posting. Address client questions regarding postings as appropriate per guidance from TAP staff. 3.1.5 Developing and maintaining inventories of computer software, books, periodicals and other job search related materials housed in the Resource Room. 3.1.6 Maintaining Resource Room book checkout log and contacting clients to locate and reclaim overdue materials. 3.1.7 Providing information and referral assistance regarding FSC programs and local community career, job search or veterans resources as instructed by TAP and CFP staff. 3.18. Tracking and maintaining supplied statistics regarding client use of Resource Room services, compiling and submitting resulting data to TAP Manager on quarterly basis. 3.2 Contractor shall provide administrative support to TAP and CFP staff. This shall include: 3.2.1 Maintaining files for client Department of Defense (DD) Forms 2586, filing, tracking and mailing forms to clients, while annotating status via log or computer networked MIS system. 3.2.2 Assisting TAP staff in preparation for monthly TAP Seminar, gathering information materials and creating student packets, contacting registrants for class attendance confirmation, developing seminar critique form and name tags. 3.2.3 Utilizing Excel or other spreadsheet program to summarize TAP Seminar feedback. 3.2.4 Briefing clients on FSC Resources at TAP Seminar as needed. 3.2.5 Assisting TAP and CFP staff with coordination of bi-annual Job Fair, contacting employers to confirm point of contact and attendance and organizing materials and supplies required at site. 4.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES: 4.1 The government will provide the following facilities, equipment, and support: 4.1.1 Workspace located within the FSC, 8535 Wewak Drive, Elmendorf AFB. Contractor shall be responsible for the orderliness and cleanliness of the areas being utilized. These areas shall be kept clean and neat, free of fire and safety hazards, and unsanitary conditions. The contractor shall be responsible for safeguarding the government-furnished property. 4.1.2 Reasonable amounts of water, electricity, and other valuable utilities will be furnished while performing work under this contract. The contractor is expected to exercise conservation of base utilities. If the contractor is found to be wasting base utilities, the contractor shall be required to meter all utilities being used and shall be billed for utilities. 4.1.3 Telephones for official use only. The contractor will maintain telephone logs for all long distance calls by date, party called, phone number, and purpose of the call. 4.1.4 Access to computers, printers, hardware, software and other materials and equipment necessary to maintain and operate both stand alone computers and those linked to the base server. 4.1.5 Keys to access Bldg. 8535. The contractor shall establish and implement a method of ensuring keys issued by the government are not lost or misplaced and are not used by unauthorized personnel. The contractor shall immediately report to the Quality Assurance (QA) personnel or Contracting Officer any occurrence of loss, unauthorized use, or unauthorized duplication of keys. In the event keys are lost or duplicated, the contractor may be required to re-key or replace the affected lock or locks without cost to the government. 4.1.6 Initial orientation training on FSC programs and services, followed by monthly training/meetings thereafter to maintain currency of information and services provided. 5.0 HOURS OF OPERATION: 5.1 The contractor shall perform the services required under this PWS during the following hours: Monday through Friday, for thirty-five (35) hours per week. The government will provide the contractor a monthly schedule of hours needed to support the requirements of this PWS, seven (7) days before the start of each month. The contractor will be notified seven (7) days in advance of any variations to the established monthly work requirements. Recognized Federal holidays include New Year_s Day, Martin Luther King, Jr. Birthday, President_s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran_s Day, Thanksgiving Day, and Christmas Day. .
