- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation DADA13-00-T-0164 is issued as a Request for Quotation. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This solicitation document and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 97-19. SIC Code is 5047and size standard of 500. This solicitation is issued as full and open competition. The Western Regional Contracting Office intends to award a firm fixed price contract for the following line items manufactured by Olympus America Inc. CLIN 0001- 1 each Cat#: TJF-160F Therapeutic Video Duoduenscope: CLIN 0002 -- 3 each Cat#: CV-160 Video Processor; CLIN 0003 2 each Cat#: CLV-160 Light Source; CLIN 0004- 1 each Cat#: CF-Q160S Flexible Sigmoidoscope; CLIN 0005 3 each Cat#: GIF-160 Video Gastroscope; and 2 each operator's manuals; 2 each service maintenance manuals. Ship to address is Madigan Army Medical Center, Materiel (Property Book), Receiving Section, Building 9040, Door 2, Tacoma, WA 98431. Request FOB Destination pricing. The following FAR clauses and provisions apply to the acquisition: 52.212-1 Instructions to Offerors-Commercial Items; however, sub-paragraphs (g) and (h) are deleted as this is an RFQ. FAR 52.212-2 Evaluations-Commercial Items. The following factors are included in FAR 52.212-2(a) and will be used to evaluate offers: Technical capability, Past Performance and Price. Technical capability and past performance when combined are significantly more important than price. Offers shall submit a statement of their capability as it relates to the requirements set forth in the specifications; a minimum of two (2) references with whom past performance can be verified to include points of contact, telephone numbers and a total contract price. FAR 52.211-6, Brand Name or Equal applies to this solicitation. FAR 52.212-3 Offeror Representations and Certifications-Commercial Items; DFARS 252.212-7000, Offeror Representations and Certifications Commercial Items (all offerors are to include with their response a completed copy of FAR 52.212-3 and DFARS 252.212-7000). Further, offers must contain all commercial terms, conditions and provisions under which the offeror conducts business in the commercial marketplace, and warranty terms and conditions. Offers that fail to furnish required representations or information, or reject the terms and conditions of this solicitation may be excluded from consideration. The contractor shall ensure products provided under this contract to include hardware, software, firmware and middleware, whether acting alone or combined as a system are Year 2000 compliant as defined in FAR Part 39. Offers must provide as a minimum: 1) RFQ number; 2) Name, address, and telephone number of point of contact of offeror; 3) CLINS 0001 through 0005 Price and discount terms; 4) A completed copy FAR 52.212-3 Representations and Certifications; 5) Sufficient technical literature to allow the Contracting Officer to evaluate; 6) Provide a minimum of two (2) current references for past performance information to include point of contact name, telephone number and type of supply for whom contracts have been satisfactorily completed. Offerors are responsible for accurate addresses and telephone numbers for submitted past performance information. Basis of Award will be on evaluation factors related to technical capability, price and past performance. THIS IS A BEST VALUE AWARD. The Government reserves the right to award to other than the lowest priced offeror. You must submit along with your offer, documents, which clearly describe the equipment, so that it can be evaluated. Equipment not clearly described will not be considered. Equipment otherwise equal may not satisfy the requirement due to differences in environmental application, etc.
