- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-19. 4RP 613-60-00 should be referenced on all proposals. The Standard Classification (SIC) for this procurement is 8099 and the small business size standard is gross annual receipts NOTE $5.0 million. This procurement is unrestricted. The VA Medical Center, Route 9, Martinsburg, WV 25401, has an emergent need for two (2) Temporary Locum Tenens Board Certified/Eligible Gastroenterologists to provide GI management services to veteran patients and eligible beneficiaries. Candidate may be a Board Eligible Physician if within (4) years of postgraduate residency and/or internship training. Services shall be required 40 hours per week, Monday through Friday, 8:00 a.m. to 4:30 p.m., excluding Holidays. Gastroenterologists shall not be required, except in case of emergency, to furnish such services on a National Holiday or during off-duty hours. VAMC Martinsburg, WV intends to solicit proposals and award a firm, fixed-price contract under the authority of Title 38 USC Section 8153. The contract is for a one hundred twenty (120) day period but may be extended for an additional three month period. Gastroenterologists shall be a citizen of the United States or hold a valid visa (green card), hold a valid and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or the District of Columbia, and shall be proficient in the English language, both spoken and written. The Gastroenterologists shall perform the following functions at this facility: (1) Conduct gastroenterology clinic services; (2) Endoscopy procedures; (3) Responsible for inpatient GI consults; (4) Responsible for the dictation of clinic operation notes; (5) Diagnose within the scope Gastroenterology; (6) Treat within the scope of Gastroenterology; (7) Peform: (a) Colonscopy; (b) Conscious sedation; (c) Endoscopic control of bleeding; (d) ERCP diagnostic and therapeutic; (3) Esophageal vaaricus injection ligation; (f) Esophago-Gastroduodenscopy with biopsy; (g) Liver Biopsy; (h) flexible sigmoidoscopy. Offerors shall submit the following with their proposals: pricing schedule, curriculum vitae, medical license (from a State, Territory, or Commonwealth of the United States or the District of Columbia), evidence of TPA Certification, and evidence of Indemnification and Medical Liability Insurance. Offerors may propose more than two (2) candidates. The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following: Worker's Compensation Insurance, Professional Liability Insurance, Income Tax Withholding, Social Security payments, Health Examinations and, ACLS and BLS Certification. The parties agree that such personnel shall not be considered VAMC employee(s) for any purpose and shall be considered employee(s) of the Contractor. The Contractor shall perform required services in accordance with established principles and ethics of the medical profession established by the AMA, ACEP, JCAHO, The eterans Health Administration (VHA) regulations, and the policies, procedures, and regulations of the Medical Staff bylaws of the VAMC, Martinsburg, WV. Contractor personnel shall provide proof of the following test within five (5) calendar days after award and prior to the first duty shift. Tests shall be current within the past year. Tuberculosis Testing: All Contractor personnel shall provide proof of a negative reaction to Purified Protein Derivative (PPD) testing. A negative chest radiographic report for activ tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually. Rubella Testing: All Contractor personnel shall provide proof of immunization for measles, mumps, rubella or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella imunization must be administered with follow-up documentation to the Contracting Officer's Technical Representatiave (COTR). OSHA Regulation Concerning Occupational Exposure to Bloodborne Pathogens: The Contractor shall provide a generic self study training module to its personnel; provide Hepatitis B vaccination series at no cost to its personnel who elect to receive it; maintain and distribute an exposure determination and control plan to its personnel; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident. VA will notify the Contractor of any significant communicable disease exposures as appropriate. THe Contractor's occupational health provider shall adhere to current CDC/HICPAC Guidelines for Infection Control in health care personnel (AJIC 1998; 26:289-354) for disease control. The Contracting agency shall provide follow-up documentation of employee's clearance to return to the workplace prior to their return. Offerors shall provide a completled set of provisions with their offer found at FAR 52.212-3 -- Offeror Representation and Certifications Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4 and 52.212-5 apply. The following FAR and VAAR claues and provisions are added by addendum: 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 852.237-7 Indemnification and Medical Liability Insurance; 852.270-4 Commercial Advertising; and 852.271-70 Services Provided Beneficiaries. Proposals shall be accepted immediately, but no later than September 27, 2000. Due to the critical need of these services, award may be made prior to September 27, 2000. An official authorized to bind the offeror's organization shall be required to sign all offers.
