Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. Standard industrial classification code 5047, size standard 100 employees, applies to this procurement. The following brand name or equal system should be delivered F.O.B. destination to the Chief, AMM&S Service (90P), VA Medical Center, 2002 Holcombe Boulevard, Houston, Texas 77030. Delivery should be made within two (2) weeks of receipt of order. Item #1:Size Wise Bari-Rehab Platform, Cat. #SW4880, 48" x 80" Platform size; 1000 lb. weight capacity; High-low ranges from 13" low deck to 30" high deck; Trendelenburg/Reverse Trendelenburg function; modified cardiac lounge position; traction/trapeze capability; removable three (3) position side rails, 45 degrees and down or equal, 3 each. Item #2: Size Wise Bari-Lift and transfer System, Cat. #SWTC1000, 1000 lb. Weight capacity to include SSL sling with hardware; telescoping height adjustable from 78" to 93" or equal, 3 each. Item #3: Operating and Service manuals for both Item #1 and Item #2. In order to be acceptable, an equal system must be compatible in every way. The provision at 52.212-1 Instructions to Offerors Commercial, applies to this acquisition. Offers will be evaluated on price and the technical capability of the item offered to meet the Government's requirement. Offerors should include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items and of 852.219-70 Veteran-Owned Small Business. Clause 52.212-4 Contract Terms and Conditions -- Commercial Items applies to this acquisition as well as the following addenda. 852.211-70 Service Data Manual; 852.211-71 Guarantee -- one (1) year, and 852.211-77 Brand Name or Equal.
