Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-00-T-0129 is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-19. This is a request for the following: CLIN 0001: One (1) each of an Ultrasound Training Simulator, PN: MedSim USA Inc. UST100, to include the following specifications: UltraSim Unit with Control Panel and Cabinet; Computer Applications; 15 inch Color Monitor; Transducer (simulates 3.5 MHz) PN: TRCV; Mannequin Full-Torso (for women) PN MQFT; Portable Mannequin Table PN: TBAL; UltraSim User's Guide; Gynecology Simulation Operating Software, Gynecology CD-ROM Educational Curriculum Module; Gynecology Instructor Manual; On-site Full Installation and System Training Session; Abdominal Application Software with 20 Simulations on CD-ROM PN ABAS01; Abdominal Clinical Education Module 1 (Normal and Common Pathologies) PN ABCE02. CLIN 0002: One (1) each MedSim Laser Printer, PN: MedSim PRHP. CLIN 0003: One (1) each Mannequin Upper Torso, PN: MedSim MQUT. CLIN 0004: One (1) each Mock 7.5 MHz Linear Transducer, PN: MedSim TRL1. CLIN 0005: One (1) each Abdomen Clinical Education Module 2 (Difficult Cases and Pathologies) 20 cases on CD-ROM, PN: MedSim ABCE02. CLIN 0006: One (1) each Abdomen Clinical Education Module 3 (Ultrasound in the Acute Setting), 10 cases on CD-ROM, PN: MedSim ABCE03. CLIN 0007: One (1) each Breast Clinical Education Module (Normal and Common Pathologies), 12 cases on CD-ROM, PN: MedSim BRCE01. CLIN 0008: One (1) each Vascular Application Simulation Software, PN: MedSim VSAS01. CLIN 0009: One (1) each Vascular Clinical Education Module 1, 16 cases on CD-ROM, PN: MedSim VSCE01. Products will be shipped to: Naval Hospital Camp Pendleton, Receiving, Bldg H-135, M/F: Breast Care Health Center (Code 10A), MCB, Camp Pendleton, CA 92055-5191. This acquisition incorporates the following FAR clauses: 52.212-1 Instructions to Offerors-Commercial Items (Oct95); 52.212-3 Offeror Representations and Certifications-Commercial Items (Oct95) must be completed and submitted along with quote.
