- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation. Request for Quotations are being requested and a written solicitation will not be issued. The request for quotation number is F08602-01-T5106. This solicitation incorporates provisions which are in effect through Federal Acquisition Circular 97-15. This procurement is 100% set-aside for small business concerns. The Standard Industrial Code (SIC) is 7299. The small business size standard is $5,000,000. The performance period is 01 October 2000 _ indefinite. A blanket purchase agreement will be issued. Interested offerors shall provide as a response to this solicitation: (1) a resume detailing relevant training and experience; (2) a completed price list; and (3) a completed clause 52.212-3. Quotes may be submitted hard copy or electronically, via e-mail or fax. Regardless of method of delivery, it is the offeror_s responsibility to ensure receipt of the quote by the time and date stated on the solicitation. STATEMENT OF WORK: Organist (0930 Protestent Service), Base Chapel, MacDill AFB, Florida. Need: A positive, self-directed and motivated individual with the breadth and depth of knowledge necessary to accomplish the weekly support of the Music Program for the 0930 Service. This person must demonstrate a familiarity with and a commitment to the Christian faith. Applicants will be evaluated based on a submitted resume and references, as well as the results of an interview to be conducted by the Senior Protestant Chaplain or his/her designate. Education and Experience: The organist must have (1) a Degree in Music, (2) ten years experience as an organist. However, in depth experience in the field of music, particularly as an organist will compensate for the educational requirement. Character: Possesses the ability to: (1) work unsupervised in a fluid and sometimes-stressful environment with staff, parents and children. (2) Foster healthy, functional interpersonal relationships with co-workers and customers. (3) Maintain professional standards of conduct and public decorum including, but not limited to, dress and grooming. Skills: Interpersonal: Must possess the ability to constructively work with chapel and parish staff, customers, and other chapel musicians. Professional: Must be able to efficiently organize and manage diverse program elements. Communication: Must be able to communicate the needs and goals of the Service Music Program to chapel staff, volunteers, and customers. Musical: Must be proficient at keyboard support and be able to sight-read music. Must be open to adaptation and be well versed in a variety of musical styles, particularly those in the traditional, contemporary and gospel traditions. TASKS TO BE PERFORMED: Provides musical accompaniment to all rehearsals and performances for the MacDill AFB Chapel 0930 Protestant Worship Service adult and youth choirs. Attends weekly rehearsals and plays music for the appropriate choir. Provides all musical accompaniments to the adult and/or youth choir at the 0930 Protestant Worship Service. Plays all music accompaniments for the 0930 Protestant Worship Service, including all special services. This statement of work will include 52 worship services, 52 rehearsals, and 16 special services (i.e. memorials, Christmas, Easter, etc.) for a total of 120 units. Maintains a good rapport with the choir director and choir members. Tracks, maintains and submits the Non-Personal Services Contract Invoice to the Pastor of the 0930 Protestant worship service. Ensures that keys, if any, issued to him/her by the chapel staff, are not lost or misplaced and are not used by unauthorized persons. These keys are the property of the US Government. The organist will not duplicate any US Government key. Observes all safety and security requirements at the facility that is being used and makes all reasonable efforts to protect US Government property from damage. The contractor will provide any contractor specific supplies and equipment to include: Personal transportation. Contractor preferred, non-chapel, supplies and office equipment. The contractor will provide the personal skills, training, competency, attitude and spiritual strength necessary to meet all the requirements contained in this statement of work. The provision at FAR 52.212-1, _Instructions to Offerors _ Commercial Items_ is applicable to the solicitation and the blanks in (a) are completed as follows: price. Offerors shall include with their offers a completed copy of the provision at FAR 52.212-3 _Offeror Representations and Certifications_ Commercial Items_ FAR 52.212-4, Contract Terms and Conditions_ Commercial Items is applicable to this solicitation. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders_ Commercial Items is applicable and the following clauses are checked: 52.222-21 _Prohibition of Segregated Facilities_; 52.222-26 _Equal Opportunity_; 52.222-35 _Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era_; 52.222-36 _Affirmative Action for Workers with Disabilities_; 52.232-33 _Payment by Electronic Funds Transfer_ Central Contractor Registration_. FAR 52.232-18 _Availability of Funds_; and 52.219-6 _Total Small Business Set-Aside_ are applicable. DFARS 252.204-7004 _Required Central Contractor Registration_ (CCR) is applicable to this solicitation.
