Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. 541-008-01 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. the SIC Code is 3429 and the small business size standard is 500. Contractor shall provided on site maintenance at the VA Medical Center, 10000 Brecksville Road, Brecksvile, OH 44141; VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106; VA Outpatient Clinic, 221 Third Street, S.E., Canton, OH 44702; and VA Outpatient Clinic, 2031 Belmont Street, Youngstown, OH 44505. Contractor shall provide pricing for the Base Year commencing October 1, 2000 through September 30, 2001 with two one-year pre-priced option years. Services shall include a minimum of one (1) preventative maintenance inspection and test during each three-month period at all locations with the replacement of all parts, labor, and materials due to normal wear, including battery replacements and all emergency calls, seven (7) days a week, twenty-four (24) hours per day with a response time within four hours after service call is placed. Normal work hours at all VA Medical Centers are Monday through Friday, 8:00AM to 4:30PM, excluding Federal Holidays. For after hour calls, Contractor shall contact the Security Service for access to pharmacy lock areas. For the Brecksville Unit, Contractor shall report to the Engineering Office Trailer located between Bldgs. 44 and 45 (or Security Service after hours)and for the Wade Park Unit, Contractor shall report to Engineering Service, Rm. E-29 (Basement). Before starting work; obtain initials of opertor when job is completed; return service report before leaving station. For Canton and Youngstown Outpatient Clinics, contractor shall report to Security Service before beginning work. When service is completed, Contractor shall leave a service report with either Pharmacy or Security Service who shall forward the report to the Contracting Officer's Technical Representative (COTR), George Nemec, Electric Shop Supervisor, at (440) 526-3030, Ext. 6194. The provisions at FAR 52.212-1, Instruction to Offerors-Commercial Items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offer Representations and Certifications-Commercial Items, with their offer. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall inclde the following clauses: 52.203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26, 52.222-35, 52.222-36, 52.222-37 and 52.25-3. The following standard FAR and VAAR clauses are incorporated and apply to this acquisition: 52.215-17, 52.217-8, and 52.217-9 (which provides that the Government may extend the term of the contract within 60 calendar days, provided that the Government shall provide a preliminary notice of intent at least 60 calendar days prior to the expiration date; and services under this contrat shall not exceed five (5) years, 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer), 52.244-6, 52.252-2, and VAAR 852.219-70, 852.237-70, 852.270-1 and 852.270-4.
