Description

This is combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation. F31601-01-Q0070 is a request for quotations. Quotes are being requested and a written solicitation will not be issued. The applicable Standard Industrial Code (SIC) is 8049. This requirement is for a Full-time Dietitian to work at Pope AFB_s Health and Wellness Center from 1 Oct 00 through 30 Sep 01. The Dietitian will work Monday through Friday, 7:30 am -- 4:30 pm, with one (1) unpaid hour for lunch. There is a possibility that after hours work may occur. Please provide your quote with an hourly date based on 2,080 hours for the year, then the total price. Please provide a Resume with your quote. The following clauses/provisions are applicable: FAR 52.212-1, Instructions to Offerors-Commercial Items, a completed copy of FAR 52.212-3, Offeror Representation and Certifications-Commercial Items, FAR 52.212-4, Contract Terms and Conditions-Commercial Items, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, paragraphs (b)(12), (13), (14), (15), (23). Also, paragraphs (c) (1) and (2). Contractors must be registered in the Central Contractor Registration system to be eligible to receive this award. All quotes with Resumes are due not later than close of business, 4:15 pm, eastern standard time, on 28 Sep 00. The information can be faxed to 910-394-6279. Contractors are responsible for verifying receipt of faxed quotes and Resumes.
