Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Format in Subpart 12.6, as supplemented with additional information included in this notice. This annoncement constitutes the only solicitation; quotations are being requested and a written Solicitation will not be issued. Solicitation Number F04626-00-QY123 is being issued as a 100% Small Business Set-Aside Request Quotation(RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. This acquisition falls under Standard Industry Classification 4959; the size standard is $4 million. This solicitation contains only one line item as follows: 0001 "The contractor shall provide everything necessary to clean and sanitize 42 portable toilets at Travis AFB. The contractor shall conform to the requirements, and comply with industry standards and all applicable laws and codes in performance of this work. Work includes but is not limited to the following: The contractor shall clean and pump wastes from 42 portable toilets; replenish toilet tissue in portable toilets; and report problems with portable toilets to the 60th Civil Engineering (60 CES) Utilities Shop foreman, or designated representative, Building 806, extension (707) 424-3025. Work shall be performed weekly, on Wednesdays, between 7:00 am and 4:00 p.m. Deviations from this schedule will be approved by the 60 CES Utilities Shop foreman, or designated representative. The contractor shall report to the 60 CES Utilities Shop foreman or designated representative to receive listing of toilet locations, prior to beginning work. Period of Performance is 01 Oct 00 through 30 Sep 01 plus three option years. Quantity: 12 Months. Perform services as indicated above for Option #1 For the Period of 01 Oct 01 through 30 Sep 02. Quantity: 12 Months. 0003 perform services as indicated above for Option #2 For the Period of 01 Oct 02 through 30 Sep 03. Quantity: 12 Months. 0003 Perform services as indicated above for Option #3 For the Period of 01 Oct 03 through 30 Sep 04. Quantity: 12 Months. Preferred FOB point is destination/ Travis AVB, CA. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition without addenda. FAR 52.212-2 is not applicable to this acquisition. Offerors must include a completed copy of FAR 52.212-3 Representation and Certifications -- Commercial items with this offer. FAR 52.212-4, Contract Terms and Conditions- Commercial Items to this acquisition without the addenda. Wage Determination No. 94-2069 Revision No. 4, Date of Last Revision 06/04/2000 applies in full text. DPAS rating for this acqusition is S10.
