Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-19. RFP 658-10-01 should be referenced on all proposals. The Standard Classification (SIC) for this procurement is 2834 and the small business size standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Medical Center, 1970 Roanoke Boulevard, Salem Virginia 24153, has an emergent need for the following Nuclear Medicine Pharmaceuticals. Quantities specified represent the Governments best estimates. However, actual requirements may be more or less than specified. In any event, contractor shall provide the amounts required at the price bid. CONTRACTOR SHALL PROVIDE UNIT PRICE, TOTAL PRICE PER LINE, AND TOTAL YEAR ESTIMATE. Contract period shall be for the period October 1, 2000 through September 30, 2001. Supplies shall be ordered as required. Daily standing orders are to be delivered to Nuclear medicine Department between the hours of 6:50am and 8:00am. Contractor shall provide up to forty-five (45) deliveries per month for the price bid. It is anticipated that an average of two (2) deliveries per day shall be required. However, should there be a requirement for more or less than the estimated deliveries on a given day, no additional charges shall be made to the Government until the monthly total exceeds forty-five (45) deliveries, except for after hours deliveries. Contractor shall deliver routine items Monday through Friday during normal business hours of 8:00am to 4:30pm. Deliveries shall be made to Nuclear Medicine Department and/or Endocrine Lab. Emergency deliveries shall be made within ninety (90) minutes after receipt of order. Contractor shall furnish a copy of shipping invoice with each delivery to the Joanne Hines (114), Nuclear Medicine and/or Chief, Endocrine Section, Medical Service, as appropriate. Deliveries shall be F.O.B. Destination, except after hour/Holiday deliveries or when deliveries exceed forty-five (45) per month. Deliveries shall not be made on Saturdays, Sundays or Holidays, except in cases of extreme emergencies. Current MSDS sheets shall be provided for all items required under the contract. Offerors shall provide a completed set of provisions with their offer found at FAR 52.212-3 -- Offeror Representation and Certifications Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4 and 52.212-5 apply. The following FAR and VAAR clauses and provisions are added by addendum 52.217-8 Option to Extend Services, 52.223-7 Notice of Radioactive Materials, 52.232-18 Availability of Funds, 852.216-70 Estimated Quantities for Requirements Contract, 852.219-70 Veteran-Owned Small Business, 852.237-70 Contractor Responsibilities, and 852.270-4 Commercial Advertising. Offerors may find the FAR provisions and clauses on the Internet at www.arnet.gov/far/. Proposals shall be accepted immediately, but no later than September 29, 2000. An official authorized to bind the offeror's organization shall be required to sign all offers.
