Description
This is a combined synopisis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. Standard industrial classification code 5048 applies. VA Medical Center, 500 West Fort St., Boise, ID is in the process of establishing a full surface Ophthalmic Laboratory. An estimated 200 eye glasses will be manufactured daily. We are looking for the following items: (1) Surfacing Equipment: one each non-alloyed blocker, one each line dryer, three each cylinder machines, one each pad press, two each lens/lap cabinet -- seven drawers, one each surface generator (dry cut), one each lab management system, and three each bar code readers; (2) Surfacing Accessories: one each slab off kit, 309 job trays -- different colors, one month worth of supplies (blocks 4, 6, 8 base), six each pad dispenser, one each sag gauge cylinder, and one each sag gauge sphere; (3) Surfacing Supplies (one month) polish-plastic-sheen, pads; (4) Coating Equipment: one each lens coating system; (5) Finishing Equipment: one each deblocker, one each glass lens edger, one each frame tracer, one each polishing lends edger, one each remote order software, one each nine tint lens system, one each computer tint-gradient, and one each deluxe frame warmer; (6) Finishing Accessories: one each bar code reader for tracer; (7) Finishing Supplies(one month) block large white vario x 200, block half eye vario x 20, leap II pads (2000) x 4; (8) Laps: lap (plastic 2.00 base) x 50, lap (plastic 6.00 base) x 50, lap (plastic 10.00 base) x 30, and lap (finished plastic) x 906; (9) Software: remote order entry, job entry, calculation done automatically, lab management, job tracking, inventory control, and comprehensive reporting. Included with this system is installation and set-up of equipment, one year warranty, also to include training of government employees. F.O.B. destination. Operation and Service manuals for all equipment items are required. Offers are required to provide existing product literature to include specifications of equipment prior to award.
