Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-00-T-0130 is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-19. This is a request for the following: CLIN 0001: Centrysystem-3 Blood Tubing Cartridges, 15/CS, Mfg/PN: Cobe Laboratories 003-410-510-C3, quantity: 84 CS, twenty one (21) cases to be delivered on 10/2/00, 1/2/01, 4/2/01, 7/2/01; CLIN 0002: Sodium Bicart Column, 10/CS, Mfg/PN: Cobe Laboratories Bicart-10/720gm Column, quantity: 124 CS, thirty one (31) cases to be delivered on 10/2/00, 1/2/01, 4/2/01, 7/2/01; CLIN 0003: Dialysate Concentrate, 4/CS, Mfg/PN: Cobe Laboratories AC2409-4 (2.0K/2.5Ca), quantity: 400 CS, one hundred (100) cases to be delivered on 10/2/00, 1/2/01, 4/2/01, 7/2/01; CLIN 0004: Potassium Chloride Dialysate Additive, 125/BX, Mfg/PN: Cobe Laboratories KCL 9GRAMS, quantity: 40 BX, ten (10) boxes to be delivered on 10/2/00, 1/2/01, 4/2/01, 7/2/01; CLIN 0005: Calcium Chloride Dialysate Additive, 200/BX, Mfg/PN: Cobe Laboratories CaCL2.2H20 4.4GRAMS, quantity: 32 BX, eight (8) boxes to be delivered on 10/2/00, 1/2/01, 4/2/01, 7/2/01; CLIN 0006: Ascorbic Acid Dialysate Additive, 250/BX, Mfg/PN: Cobe Laboratories 0.18GRAMS, quantity: 8 bx, two (2) boxes to be deliveredon 10/2/00, 1/2/01, 4/2/01, 7/2/01.
