This is a modification to the original synopsis/solicitation. This is a IFB. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 19. Delivery to be within 30 days from date of award. If FOB is not destination, freight charges must be noted in offeror bid. Item(s) are to be delivered as follows: MR contrast dye injector to Warehouse, VA Medical Center, 508 Fulton Street, Durham, NC 27705; CT contrast dye injector to Warehouse, VA Medical Center, 2300 Ramsey Street, Fayetteville, NC 28301; Angio contrast dye injector to Warehouse, VA Medical Center, 1201 Broad Rock Boulevard, Richmond, VA 23249. Offerors are to include with their offer a completed copy of the provision at 52.212-3, "Offeror Representation and Certification -- Commercial Items." Provision 52.212-2. "Evaluation-Commercial Items," and Clause 52.212-4. "Contract Terms and Conditions-Commercial Items" and 52.212-5, "Contract Terms and Conditions Required to Implement Statutes or Executive Order -- Commercial Items." are applicable to this acquisition.
