Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number: RFQ 523-12-01. This solicitation document and incorporated provisions are those in effect through FAC 97-19. Contractor shall provide the following opthomalic equipment or equal to the VA Medical Center, 150 South Huntington Avenue, Boston, MA 02130-4893. Requirements: Ten (10) each Haag-Streit 900 BM Slit Lamps. In order to be acceptable, an equal system must be compatible in every way. Offers will be evaluated on price and the technical capability of the item offered to meet the Government's requirement. Required delivery date shall be 60 days after receipt of award. Submit signed and dated quotations on company letterhead. Offerors should also include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items and of 852.219-70 Veteran-Owned Small Business. Clause 52.212-4 Contract Terms and Conditions -- Commercial Items applies to this acquisition as well as the following addenda. 852.211-70 Service Data Manual; 852.211-71 Guarantee -- one (1) year, and 852.211-77 Brand Name or Equal.
