Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Acquisition of Commercial Items and Subpart 13, Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number RFQ 600-002-01 is being issued as request for quotation (RFQ) which incorporates provision and clauses through Federal Acquisition Circular 97-19. The NAICS code 514120 applies to this solicitation. The Government intends to solicit and negotiate with CPR Technologies for a Plato Software version, pda v2.7 Base application installation, training, support and maintenance. The Plato software is targeted specifically to serve the needs of health care information in Joint Commission Health Care Organization Standards. This product will enable VA Greater Los Angeles Health Care System to provide timely and complete medical records review as delineated in the standard and Network within the VAGreater Los Angeles Health Care Center West L.A Campus, Sepulveda Campus and Los Angeles Ambulatory Care Center. Line Item One : (PDA V2.7) Base Application installation software and training (15 users, 5 for each facility) located at West Los Angeles, Sepulveda Campus and Los Angeles Ambulatory Care Center. Line Item Two: Support and Maintenance (15) months, including Software and User Licenses. The following Federal Acquisition Regulation (FAR) and Veterans Affairs Acquisition Regulation (VAAR) clauses and provisions identified herein may be accessed electronically at these addresses http://www.arnet.gov/far and http://www.va.gov/oa&mm/Vaar. The following incorporated clauses and provisions are applicable to this solicitation: FAR clause 52.212-1, Instructions to Offerors-Commercial Items (Oct 2000) is hereby incorporated by reference, the offer must include a copy of the standard commercial warranty per FAR clause 52.212.1, (b.5), Terms of any Express Warranty. FAR clause 52.212-4, Contract Terms and conditions-Commercial Items, is hereby incorporate by reference (May 1999). FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (Aug 2000), is hereby incorporated by reference. This notice of intent is not a request for competitive proposal. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. However, responsible interested parties may submit a quotation in which identify their capability and price no later than fifteen (15) calendar days from date of publication of this announcement. Note: 22 applies.
