Description
This combined synopsis/solicitation for psychological services in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. Proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only the only solicitation; proposals are being requested and no written solicitation will be issued. This is a 100 per-cent set-aside for American Indian contractors. See numbered note (1). Sic Code is 8011. Size standard is 500. This solicitation identified as RFQ-00-1 is issued as a Request for Quote. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Regulations (FAR). Quotes shall be in accordance with all requirements of this notice including those which are included by reference. This requirement consists of providing psychological services at Chemawa Indian School, 3700 Chemawa Road, NE, Salem, Oregon, 97305-1199, (503) 399-5721. The contractor shall perform professional work in a school educational setting, during school hours 8:00 A.M. 3:30 P.M. He/she will use practical understanding of the principles, methods, and techniques of psychology in carrying out his/her duties in the field of counseling, diagnosing and developing remedial programs for students. In service training will be given to education personnel in the psychology of working with students. This position requires the administration and interpretation of cognitive, academic, and behavioral assessments. This information is necessary to qualify students for services under Individuals with Disability Education Act (IDEA). The contractor shall be part of the Individual Education Plan (IEP) meeting when necessary to offer recommendations to the team to promote student success. The position requires the contractor be available to provide behavioral counseling, classroom interventions and other related services to students with disabilities. Individuals applying for this position must provide proof of educational training and possess a valid license as a School Psychologist. Chemawa is an off-reservation boarding school for Native American high school students. The services requested are to be provided during the school day. Contractor providers may quote to perform a portion or all of the services. The quotation may be submitted on a flat hourly rate or divided between the cost of each assessment and the hourly rate consultation services. The following provisions apply: 52.212-1 Instructions to Offerors-Commercial Items. 52.213-3 Offeror Representations and Certifications Commercial Items (must be submitted with quote). 52.212-4 Contract Terms and Conditions Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items; 52.219-8 Utilization of Small Business Concerns and Small Disadvantaged Concerns (15 U.S.C. 637 (dx2) and (3). Quotes must include information listed at 52.212-1 (b) (1-11). Instructions to Offerors Commercial Items. Quotations that fail to provide required Representations and Certifications at 52.212-3 Offeror Representations and Certifications-Commercial Items, and/or reject the terms and conditions of this solicitation may be excluded from consideration. Evaluation factor: price. Required delivery date of requirement is November 6, 2000.
