Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes the only document to be issued. A formal solicitation WILL NOT be issued. This Notice incorporates provisions and clauses in effect through Federal Acquisition 97-20. RFQ 667-10-01 should be referenced on the quote. The North American Industry Classification System (NAICS) for this procurement is 621999 and the small business size standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. Requirement: The Overton Brooks VAMC, 510 East Stoner Ave., Shreveport, LA 71101, has an emergent need for one Certified Ultrasound Medical Instrument Technologist (RDMS) to provide services to veteran patients and eligible beneficiaries. Services shall be required 40 hours per week, Monday through Friday, 8:00 a.m. to 4:30 p.m., the will not be an Call Back. Overton Brooks VAMC, intends to solicit Quotes and award a firm, fixed price contract under the authority of Title 38 USC Section 8153. The contract is for a 6 months. The VA reserves the write to reduced the term of the contract with a 30 day notice if the recruitment efforts prove successful. The Technician shall be a citizen of the United States and shall hold a valid and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or the District of Columbia and shall be proficient in the English language, both spoken and written. The Technician shall perform 8-12 Exams per day to include General ABD., Vascular to Invasive procedures. CLAUSES The provisions at FAR 52.212-1, Instructions of Offeror -- Commercial Items (Oct 2000)applies to this acquisition. There is not an addenda to these provisions. The clauses at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May 1999) applies to this acquisition. There is an addenda to this clause.
