Description
Provide professional A/E design services to include site evaluation, determination of requirements, recommendations, & design for new construction to expand the existing Myrtle Beach Outpatient Clinic, 3381 Phillis Blvd., Bldg. 334, Myrtle Beach, SC. Design will include site considerations & recommendations, structural requirements, existing & new utilities required, VA accessibility requirements, finishes, signage & applicable design guides & specifications. Effort will include planning, consultation & visits, design work, preparation of contract drawings & specifications, preparation of cost estimates & construction period services as necessary for demolition, construction, inspection, testing & certification & as-built documentation for Project 534-311, Expand Outpatient Clinic -- Myrtle Beach, SC. Final plans & specifications are required to be complete & ready for issue for bidding by June 11, 2001. NAICS: 54131. Estimated construction cost: $1,314,000. Project is intended to expand the existing outpatient clinic by the addition of approximately 5,300 square feet of new construction capable of a future second floor, expansion of existing parking, & creation of a covered patient drop-off area. Discipline involved is primarily architectural, but will include structural, electrical, mechanical, & other disciplines as needed. All professional services shall be a part of the design team announced in the 254's & 255's. Specific evaluation factors, not in priority order shall be: 1) Team proposed for this project: Professional qualifications of the entire team in the area of Health Care Facility Design are necessary for the satisfactory performance of the required services. The team will be given particular attention for its overall coordination; previous experience in working together as a team; and estimating capabilities. Individual members of the team will be considered for their professional qualifications & experience relevant to the project. 2) Previous experience of the proposed team: Prior experience by all members of the team in the design of alterations, maintenance, & repair projects, development of conceptual designs, & preparation of construction documents including estimates & specifications. Specialized experience & technical competence by all members of the team in health care facilities, Government agencies, & the Department of Veterans Affairs facilities. 3) Proposed design approach for this project: Proposed design philosophy, anticipated problems & potential solutions. 4) Proposed management plan: Team organization for conceptual, design, & construction phase. Ability to meet project specific design deadlines. 5) Location & facilities of working offices within a 200-mile radius of Myrtle Beach, SC: Including the prime firm & consultants. 6) Other: Techniques planned to control the schedule & costs, personnel responsible for schedule & cost control, estimating effectiveness of the 10 most recently bid projects, AutoCAD Release 14 capabilities, Internet accessibility, past litigation involvement, etc. A/E firms are required to respond in writing if interested & shall submit a completed SF 254 & SF 255 to the Ralph H. Johnson Dept. of Veterans Affairs Medical Center, ATTN: Mary S. Whittaker (90CIFO), 109 Bee Street, Charleston, SC, 29401-5799. SF 254's are NOT kept on file at this VAMC. Firms selected for interview will be requested to present their approch for this project & demonstrate their experience. This is not a request for proposal.
