Description

Contractor will provide Stat Laboratory Tests (Professional Fee) on patients and/or speciments at the DVA Ukiah Community Based Outpatient Clinic (CBOC), 238B Hospital Drive, Ukiah, CA 95482 for the period of February 1, 2001 (or date of award) through January 31, 2001.These services include 1. Obtaining Laboratory Specimens, 2. Perform Laboratory Tests on Specimens and 3. Fax results of Stat Laboratory Tests to the Ukiah CBOC. The DVA estimates tghere will be an average of ten (10) test per month, although the contract will not obligate the Government to purchase any services. The Contractor will fax the results to the Ukiah CBOC within one hour of receiving a specimen. The contractor's facility must be within five minutes walking time of the Ukiah CBOC. Solicitation documents will be available o/a November 14, 2000 at the following website: http://www.va.gov/oa&mm/busopp/sols.htm. No telephone request will be accepted. No e-mail request will be accepted. Request for hard copy solicitation must be in writing and may be faxed. No web-based orders will be accepted. The offeror will be responsible for downloading all documents and amendments from this site without further notice from the DVA.
