Description

The Confederated Tribes of the Warm Springs Reservation of Oregon is seeking proposals for Maternal Child Health Services. The Tribes is looking to possibly contract for a three-year period with a Maternal Health Nurse to provide maternity case management services for approximately 20-30 hours per week. Duties and Responsibilities include: Conducting initial assessment to determine the clients current status, identify needs and strengths in physical, emotional, developmental, environment and nutritional areas to develop client service plan in order to optimize the pregnancy outcome. Coordinating MCH services with the Public Health Nursing Department of the Indian Health Services and the Early Head Start Program of the Confederated Tribes; provide referrals to other health and social service agencies in the community; Provide Nutritional Counseling; Education Services and Consultation Services with physicians.
