Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in sub-part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Solicitation F02601-00-T0408 is being issued as a request for quotation. (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 97-16. (IV) This requirement is being synopsized as a small business set-aside under standard industrial classification code 3844, size standard 500 employees. EQUIPMENT TO BE SERVICED: (V) To provide preventive maintenance on the following items: Line Item 1: X-rat unit, Rad/Flouro, model #ADVANTX. Line Item #2: X-Ray unit, Rad/Flouro, model #MVP80. LINE item #3: Mammography unit, model #SENOGRAPHE DMR. The government reserves the right to award to the _other than low_ offeror, if it is in the Government_s best interest. (VII) Extent of obligation will be from 1 Sep 00 _ 30 Oct 01. Place of performance: DRMS-UH 7030 E. Irvington Rd. Davis-Monthan AFB, AZ 85707. (VIII) Provision 52.212-1 Instructions to Offerors- Commercial Items applies. (X) Offerors are required to submit a completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items with its offer. (XI) Clause 52.212-4 Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5 Contract Terms and Conditions Require to Implement Statutes or Executive Orders-Commercial Items applies to the award document. (XVI) Quotations are due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, by 17 November 2000, 4 p.m. local time. A site visit may be requested on an individual basis. Arrange this with the POCs listed in XVII.
