Description

The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to provide Full Service/Maintenance for Acuson Sequoia C256 Ultrasound Device and related components to include parts, labor, and technical assistance for the Department of Veterans Affairs, Central California Health Care System, 2615 E. Clinton Avenue, Fresno CA 93703, a Department of Veterans Affairs facility within the Veterans Integrated Service Network (VISN 21). Contract period is December 1, 2000, or date of award, through November 30, 2001 with four one-year renewal option periods through November 30, 2005.Contract award is subject to availability of funds. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors Commercial Items; 52.212-2 Evaluation Commercial Items; 52.212-3 Offeror Representations and Certifications -- Commercial Items; 52.212-4 Contract Terms and Conditions Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders Commercial Items.
