- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This solicitation is being issued as a Request for Quotation (RFQ) and is being conducted under the Simplified Acquisition Procedures (FAR Part 13) This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation reference number is RFQ 531-08-01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20 and Veterans Affairs Acquisition Regulation (VAAR), and VA Circular 97-8. Federal Acquisition Regulation (FAR) clauses and provisions incorporated herein by reference are FAR 52.212-1 Instructions to Offerors-Commercial (OCT 2000), 52.212-2 Evaluation Commercial Items (JAN 1999), 52.212-3 Offeror Representations and Certifications-Commercial (OCT 2000), 52.212-4 Contract Terms and Conditions- Commercial Items (MAY 1999), 52.212-5 Contract Terms and Conditions required to implement Statutes or Executive Orders-Commercial Items (AUG 2000), 52.222-21, 52.216-18, 52.216-19, 52.216-21, 52.217-8, 52.217-9, 52.232-34, 52.233-2, and VAAR 852.216-70 and 852.233-70. This acquisition is 100% set aside for Small business. The NAICS code is 421460, size standard is 100 employees. The Department of Veterans Affairs Medical Center, Boise, Idaho anticipates establishing an in-house eyeglass manufacturing laboratory to provide eyeglasses to eligible beneficiaries of the VA Medical Centers of Veterans Integrated Service Network (VISN 20). This requirement is for the purchase of eyeglass frames only. The following frame types are required: 5 men's metal, 3 men's zyle, 2 ladies zyle, 3 ladies metal, 2 unisex (1 deep, 1 oval) and 2 halfeye (1 metal, 1 zyle). The following requirements shall apply: (1) Eyeglass frames must carry a one (1) year manufacturers warranty. (2) All frames must arrive completely assembled. (3) All orders from the manufacturer must represent a complete order. (4) All orders must be shipped within 24 hours of the receipt of the order, emergency orders are to be received at the VA Medical Center, Boise, Idaho within 48 hours. (5) Contractor must notify the Boise VAMC within 24 hours of any removal, recall or modification of frames. And, (6) Frames shall be in accordance with American National Standard requirements for Dress Ophthalmic Frames ANSI Z80.5-1979, or latest edition whichever is most current. It is estimated that approximately 200 frames and cases will be required daily (52,000 per year). Cushioned leather-like, open-end pocket, clip-on cases must be available for purchase in case lots. Sample kits of all frames will be required for purchase by all of the Department of Veterans Affairs Medical Centers in VISN 20. Offer to provide pricing for all frame styles, sample kits and eyeglass cases. All offers must be at the designated site by 4:30 PM, November 17, 2000. Faxed offers will not be accepted. Evaluations will be made on price, past performance and technical qualifications. Offeror to provide at least 3 references with offer. All offers shall be sent to Linda L. Liquin, Contracting Officer (531/90C), A&MM Service, Building 34, 500 West Fort Street, Boise, ID 83702-4598 and must include completed FAR Provision 52.212-3 "Offeror Representations and Certifications as well as a sample of all frames that will be provided. Sample frames will be returned to contractor after completion of evaluation. Solicitation will be subject to availability of funding. Award of a firm-fixed price contract will be made on or about November 20, 2000 to the offeror whose quote conforms to the requirements of the solicitation and represents the best value to the Government, all factors being considered.
