CBD ENTRY FORM Submission Date:11-4-00 RESPONSE DATE:11-20-99 Classification: 66 Analytical System" Solicitation # RFP 600-019-01 Subject: 66 "Analytical System" Description: This is a combined synopsis/solicitation. The VA Network Business Center, Long Beach, California intends to negotiate a sole source contract with Affymetrix. The Request for Proposal # RFP 600-019-01 BV entitled "Analytical System" is hereby issued for commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 97-20. The SIC 3826 and the small business size standard is considered 500 employees. This CBD announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. Affymetrix shall provide the Analytical System to include the following line items: 0001 Affymetrix , Gene Chip Fluidics Station Mdl 400 (1 EA); 0002 Affymetrix GeneChip Hybridization Oven Mdl 640 (1 EA); 0003 Affymetrix Analytical Workstation including GeneChip Analysis Suite (1 EA); 0004 Affymetrix GeneArray Scanner (1 EA). The Laboratory Program for patient diagnostic and monitoring purposes shall use this system. Utilization of this analytical system will involve the examination of solid tumors via expression DNA to identify specific tumor cell types and examination of various genetic mutations involving both HCV and HIV viral antigens. Affymetix is the only company that provides the unique advanced methodology employed in the Analytical System that will allow for a shorter period for the laboratory to evaluate patient specimens and make a specific diagnosis. The warranty for this equipment shall be for one year. The Delivery Schedule is 30 -- 60 days after receipt of order. Delivery terms shall be FOB Destination. The product shall be delivered to the VA Greater Los Angeles Healthcare System Supply Warehouse, Los Angeles, California. The Government intends to award contract on a sole source basis to Affymetrix Offeror. Affyterix shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (FEB 2000).
