- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This solicitation is being issued as a Request for Quotation (RFQ) and is being conducted under the Simplified Acquisition Procedures (FAR Part 13) This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation reference number is RFQ 531-08-01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20 and Veterans Affairs Acquisition Regulation (VAAR), and VA Circular 97-8. This acquisition is 100% set aside for Small business. The NAICS code is 421460, size standard is 100 employees. The Department of Veterans Affairs Medical Center, Boise, Idaho anticipates establishing an in-house eyeglass manufacturing laboratory to provide eyeglasses to eligible beneficiaries of the VA Medical Centers of Veterans Integrated Service Network (VISN 20). This requirement is for the purchase of eyeglass lenses only. The following requirements shall apply: (1) All scratch coated lenses must carry a one-year warranty. (2) It is estimated that we will require 150 pairs of lenses per day and increase to 200 pairs per day after approximately 6 months. (3) All orders must be shipped within 24 hours of receipt of order and must be shipped second day air at the expense of the contractor. (4) Emergency orders are to be received at the VA Medical Center, Boise, Idaho within 48 hours (5) Contractor must notify the VA lab of short shipments as well as back order delays within 24 hours of receipt of order. All offers must be at the designated site by 4:30 PM, November 17, 2000. Faxed offers will not be accepted. Evaluations will be made on price, past performance and technical qualifications. Offeror to provide at least 3 references with offer. All offers shall be sent to Linda L. Liquin, Contracting Officer (531/90C), A&MM Service, Building 34, 500 West Fort Street, Boise, ID 83702-4598 and must include completed FAR Provision 52.212-3 "Offeror Representations and Certifications as well as a sample of all lenses that will be provided. Sample lenses will be returned to contractor after completion of evaluation. Solicitation will be subject to availability of funding. Award of a firm-fixed price contract will be made on or about November 20, 2000 to the offeror whose quote conforms to the requirements of the solicitation and represents the best value to the Government, all factors being considered.
