- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation #670-07-01 is issued as a Request for Proposals and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. This is a 100% Small Business Set-Aside. The NAICS Code is 811212 and the small business size standard is $18.0 million. This procurement is being conducted under Simplified Acquisition Procedures (reference FAR 13.5). The VA Healthcare Network Upstate New York (VISN-2) has a requirement for Cisco Systems SMARTnet Service Package with 24x7x4 Advanced Hardware Replacement on the most critical components (37 items); 8x5xNBD Advanced Hardware Replacement on remaining equipment (100 items); and 2 copies of CiscoWorks software. Hardware is physically located at Albany, Batavia, Bath, Buffalo, Canandaigua, Syracuse and various outpatient clinics throughout upstate New York. Prospective offerors can obtain the complete list of items to be included either by faxing their request to 315-477-4508 or by sending an e-mail to Sheila.Smith3@med.va.gov to receive the list electronically. The VA intends to award a contract to the offeror who will provide product support directly from Cisco at the lowest overall price. Period of coverage will be from December 1, 2000 through November 30, 2001. Responses must be in writing and can be mailed to the attention of the contracting officer, or submitted via fax or e-mail. The following Federal and VA Acquisition Regulation clauses apply to this solicitation: FAR 52.212-1, Instructions to Offerors Commercial Items (OCT 2000), with the addendum FAR 216-1 Type of Contract "The Government contemplates award of a firm fixed-price contract resulting from this solicitation"; all Offerors are required to provide a completed copy of the provision FAR 52.212-3 Offeror Representations and Certifications Commercial Items (OCT 2000), with addendum of VAAR 852.219-70 Veteran Owned Small Business; FAR 52.212-4 Contract Terms and Conditions Commercial Items (MAY 1999), with addendum of FAR 52.219-6 Notice of Total Small Business Set-Aside; and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (AUG 2000).
