- Service/Maintenance agreement for AutoMed Optifill II Prescription Packaging Machine RFQ 546-13-01 This is a combined synopsis/solicitation for commercial item services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. The VA Medical Center, Miami, Florida intends to negotiate a sole source contract with AutoMed Technologies, Inc. This announcement constitutes the only solicitation; quotations are being requested. Solicitation RFQ 546-13-01 is issued as a request for quotel and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 46. The Standard Industrial Classification Code (SIC) is 7699. Contractor shall provide all parts, labor, and emergency calls to maintain/keep AutoMed Optifill II Prescription Packaging machine up to manufacturers specifications.This service is required to be rapid and supplied parts are to be OEM. Full maintenance option shall include the following: support hours Monday Friday 8:00am 5:00pm local time. Telephone response within 2 hours or less, on-site response within 8 hrs. Semi-annual preventive maintenance inspections to be done after hours, includes both hardware and software. Technical support includes all system replacement parts. Provides 50% discount on biomedical training. Maintenance service is as follows: the period between semi-annual preventive maintenance service shall not be less than 180 days. The first PM to be performed will be Oct-Mar, with the second PM performed between Apr-Sep. Vendor will contact Biomedical Engineer (Gil Wagener) 305-324-3183 before arranging PM services. The Automed service engineer will sign the biomedical log upon entering and leaving the medical center. Service tickets will be signed by biomedical personnel upon receipt of signed service tickets, and vendors invoice will be certified for payment. The provision at 52.212-1, Instruction to Offferors-Commerical items; 52.212-2, Evaluation-commerical items, the following factors shall be used to evaluate offerers: (1) price, (2) on-site training, (3) supply of OEM parts.
