Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Format in Subpart 12.6, as supplemented with additional information included in this notice. This annoncement constitutes the only solicitation; quotations are being requested and a written Solicitation will not be issued. Solicitation Number F04626-00-QY123 is being issued as a Unrestricted Request For Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. This acquisition falls under Standard Industry Classification 7699; the size standard is $5 million. This solicitation contains two line item as follows: 0001 _The contractor shall provide everything necessary to semi annual preventive maintenance service on the Jeol Electron Microscope located at the Clinical Investigation Facility, 60th Medical Group at Travis AFB, CA_. 0002 is set aside for parts that are not covered under the preventive maintenance agreement. The contractor shall conform to the requirements, and comply with industry standards and all applicable laws and codes in the performance of this work. Work includes but is not limited to the following: The contractor shall perform two semi annual preventive maintenance service on the Jeol Electron Microscope. The contractor shall provide the necessary qualified personnel, material, test equipment, and related administrative support, including travel and per diem, to accomplish the required preventive maintenance service actions. Preventive maintenance service shall consist of, and is not necessarily limited to, the inspection, disassembly for repairs, adjustment, lubrication, replace of worn/defective parts, re-assembly after repairs, and calibration of the systems. Preventive maintenance shall be performed semi-annually. Overall, the contractor will make its best efforts to provide preventive maintenance service actions to ensure the system is functioning properly 95% of the schedule use time. Documentation of preventive maintenance shall be provide to the quality assurance evaluator (QAE), a member of the Medical Equipment Repair Center (MERC) Staff. A schedule of preventive maintenance service visits shall be submitted by the contractor to the QAE during the first 30 days of the contract period for approval of the Contracting Officer. Subsequent changes to this schedule shall be approved by the QAE and the Contracting Officer. The system shall be maintained in conformance with manufacture_s drawings, specifications, and standards, as well as in compliance with all federal and nationally accepted standards in effect. Mandatory equipment modifications specified by the equipment manufacture shall be conducted by the contractor after approval by the QAE. A copy of all documentation, i.e. work orders, schematics, diagrams, repair parts lists, service manual updates, etc, describing the modification shall be provide to the QAE. Prior to commencing preventive maintenance service, the Contractor shall advise the QAE of his presence by reporting to the MERC (first floor, West Wing) to sign in. Upon completion of services, the contractor shall report back to the MERC to sign out, and shall provide a signed copy of all service reports to the QAE. The contractor shall provide all necessary intervening repair services during normal business hours (0800 _ 1700, Monday _ Friday). A list will be provided of personnel authorized to place calls. The contractor shall respond to service call requests within two (2) working hours by telephone, and shall have a repair technician on site within sixteen (16) working hours after notification by the government. After arrival on site, the contractor shall be able to return the system to full operation within four (4) hours, within 24 hours if parts are required. The contractor average mean time to repair this instrument is sixteen (16) hours. All parts and material used shall be new or an exchange basis, and shall conform to the original manufacture_s specifications. Period of Performance is 08 Nov 00 through 30 Sep 01 plus two option years. Quantity: 1 Job. Perform services as indicated above for Option #1 For the Period of 01 Oct 01 through 30 Sep 02. Quantity: 1 Job. 0003 perform services as indicated above for Option #2 For the Period of 01 Oct 02 through 30 Sep 03. . Preferred FOB point is destination/Travis AVB, CA. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition without addenda. FAR 52.212-2 is not applicable to this acquisition. Offerors must include a completed copy of FAR 52.212-3 Representation and Certifications _ Commercial items with this offer.
