- Description
- The Naval Medical Logistics Command has a requirement for diagnostic ultrasound units for Medical Treatment Facilities worldwide. The combined synopsis/solicitation for commercial items was issued in the July 17, 2000 issue of the CBD. This is an amendment to the originally published combined synopsis/solicitation. The revisions are as follows: 1) One or more Indefinite Delivery type contract(s) will be awarded. 2) The minimum quantity requirement is 11 units. 2) The maximum quantity over a three-year period is 30 units. 3) Offers shall be evaluated in a two (2) stage process. 4) Stage 2 consists of Factor 2, Factor 3 and cost proposal submission. 5) The down select requirement has been removed. 6) The offerors who provided a demonstration to the government on 8 Aug 00 will be requested to provide the following documentation: a) Past Performance Information -- provide three references. Include the Agency name, Agency point of contact and telephone number, contract number and type and number of units provided. b) Equipment Performance Information -- Delivery after receipt of order (ARO), equipment reliability, equipment maintainability, equipment portability, and DICOM. c) Pricing on the following unit: a) Diagnostic Ultrasound Equipment; b) 2 Standard Transducers; c) Operators Manual; d) Maintenance Manual; e) Complete installation to make the unit fully operational; f) Additional Transducers -- provide listing of available transducers and price. 7) In order to determine price reasonableness, provide Commercial Price List and state discounts off of the commercial price if applicable. 8) Price evaluation and the results of Factors 2 and 3 will establish a best value award. 9) Incorporate Federal Acquisition Regulation (FAR) Clause 52.216-27 (OCT 1995), Single or Multiple Awards. 10) Offerors who demonstrated their equipment on 8 Aug 00 must submit a proposal as stipulated above by 3:00 p.m. EST, November 22, 2000.
