Description

Contractor to provide Preventive Maintenance and all parts and materials excluding glassware for the maintenance of a Multistar Top/Plus Digital Angio system (SN 1693) located in GA133A for the VA Medical Center at 50 Irving Street, NW, Washington, DC for the period of 12/1/00 9/30/01. Labor on the system to be capped at 100 hours per annum with any excess hours carrying over to remaining years in the contract. Additional hours to be provided at the prevailing rates. Vendor will provide access to Diagnostic software to be utilized by the VA for the life of the contract. Emergency callback will be within 4 hours of notification by Biomedical Engineering during the hours of 8:00am to 5:00pm, Monday through Friday excluding holidays. All responsible sources may submit an offer in accordance with the solicitation which may be obtained electronically after 11/14/00 at the following URL: http://www.va.gov/oa&mm/busopp. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. Requests for a hard copy of the solicitation must be in writing to the Contracting Officer. No telephone requests will be accepted. The NAIC code is 811219 with total annual revenues not to exceed $5 million. Awarded contract may be extended for two (2) additional option years in accordance with the clause at 52.217-9, Option to extend the term of the Contract, subject to the availability of funds.
