Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice, This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. USDA, OCFO, National Finance Center(NFC) locate in New Orleans East, LA is generating this (RFQ) for on-site confidential document destruction services the period of January 1, 2001 through September 30, 2001, with three (3) one-year renewal options. The contractor shall provide all labor, material, equipment, supplies, supervision, and transportation necessary to provide on-site document destruction services for USDA, NFC, 4432 Poche Court West, New Orleans East, LA. Services are required weekly on Monday, Wednesday, and Friday mornings. The documents are plain stock paper, carbon, envelopes, microfilm, microfiche, and diskettes. Additional services may be required during peak periods (August and September). The contractor will be provided a 24 hour notification for peak period services. Holiday and weekend pickups are not anticipated. Approximately 95% of the documents to be shredded will be plain paper stock. It's estimated that a volume of 30,000 pounds of plain paper stock will be shredded and10,000 pounds of microfilm, microfiche, and diskettes will be destroyed on a monthly basis. The contractor must remove and properly dispose of all shredded material. The contractor must be licensed and fully insured for these services. The contractor will handle all materials allowing an authorized government personnel to witness the destruction of the documents. Upon completion of the job, the contractor will provide a receipt and certificate of destruction showing pounds and number of boxes destroyed. Equipment used in collection and transportation of material to be shredded will be operated and maintained in such a manner as to minimize health and safety hazards. Documents for shredding will be located at two USDA's facilities: 4277 Poche Court West and 4432 Poche Court West. All locations are located within two blocks or less of the other. On-site shredding must be conducted in the parking lots and loading zones only. The provisions and clauses are those in effect through Federal Acquisition Circular 20. The following provisions are applicable to this acquisition: 52.212-1, Instructions to Offerors-Commercial; 52.212-2, Evaluation-Commercial; 52.212.3, Offerors Representations and Certifications-Commercial Items which must be completed and submitted with your RFQ; 52.212.4, Contract Terms and Conditions; 52.212.5, Contract Terms and Conditions Required to Implement Statues or Executive Orders; 52.217-8, Option to Extend Services; and 52-217.9, Option to Extend Term of Contract. All questions and comments must be in writing and must identify the company, contact person, E-mail address, telephone, and fax numbers.
