Description
The Immigration and Naturalization Service, Dallas, Texas is issuing a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation #**ACD-1-Q-0352 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Acquisition Circular 97-20. This is a 100% SET-ASIDE FOR SMALL BUSINESS with a standard industrial classification code of 2675 with a size standard of 500. The resulting contract will be a fixed-price, requirements type contract for the purchase of 18 PT Kraft file folders, letter size, 1/3 cut, 1st and 3rd positions only, reinforced top tab, guide height 11 3/4" x 10" with crease expansion with at least 10% post-consumer recycled materials. No printing, to be boxed by position (Do not mix). Also folder will to have a Drill ?" holes through both sides, 2 ?" centered, ?" from top. Folder may be order without holes by using CLIN 1 or with holes by using CLIN 1AA. The period of performance will be for one year with two one-year options which may or may be exercised by the Government. Delivery orders may be placed on a monthly basis to replenish stock as needed. The following estimated quantities: Contract Line Item Number (CLIN) 1. Base year 3,900,000 (approx. 50% of each cut) with an estimated 328,000 folder per month; CLIN 2. First Option Period: 1,000,000; CLIN 3. 1,000,000. The estimated use for the two option periods is 88,000 per month. An example of the type of folder required is GQ2751B manufactured by S&W Manufacturing, Inc. Your quotation must include a sample of the folder being offered. All shipments are to be FOB Destination to the specific shipping destination cited on each delivery order which will be either a facility located in Grand Prairie, TX 75050 or Dallas, TX 75227. THE OFFEROR SHALL INCLUDE A COMPLETED COPY OF THE PROVISIONS AT FAR 52.212-3, Offeror Representations and Certifications -Commercial Items, along with the firm's DUNS number. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items is applicable. Paragraph (a), evaluation criteria is as follows: Award will be made to the lowest, technically acceptable offer. Technical: Offers shall submit a technical description, pictures of the product offered in the quotation and any other information to verify conformance to specifications. Include a narrative explaining the capabilities of your firm to meet the requirements of this requirement such as financial resources, past experience in similar work, and integrity. Government past performance: Include a list of prior government contracts which includes the name and phone number of the contracting officer; name and phone number of your bank and other references Each quotation is to include a completed copy of the provisions at 52.212-3, Offeror Representations and Certifications -- Commercial Items.
