- This is a combined synopsis/solicitation. Request for Quotation RFQ 618-89-01 entitled "Urologist Services" is hereby issued for commercial services and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program; simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. This announcement incorporates provisions and clauses in effect through FAR FAC 97-20 and VAAR 97-8. NAICS Code 621111 applies for this procurement and the small business size standard is $5 million dollars. Purchase will be made in accordance with Enhanced Health Care Resources Authority 38 U.S.C. 8153. A separate written solicitation document will not be issued. Contractor's proposals are hereby being requested and response to requirements listed below shall be submitted on contractor's letterhead and must address all requirements listed below and should contain the name, title and signature of person authorized to submit proposals on behalf of the contractor. VA Medical Center (VAMC), Minneapolis, MN, has a requirement for a full-time Urologist to perform outpatient services, Monday through Friday, 8:00 a.m. to 4:30 p.m. or as coordinated with Chief, Urology. Contractor to provide malpractice insurance, salary, fringe benefits, travel and housing as necessary for a fixed hourly fee. Include overtime rates and/or on/call rates in case either of these become necessary. Period of performance is immediately for at least a 4-month period. Evaluations will be conducted to determine that the contractor employee is Board Certified and skilled in Urology services and will meet Government's requirements. Evaluation factors of technical capability include past performance history and availability along with price to determine "best value" to the Government. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead and fax immediately to Mavis Nelson, 612-725-2072.
