Description

Contractor shall provide all necessary labor, supervision, equipment, and materials to inscribe marble niche covers for National Memorial Cemetery of Arizona, 23029 No. Cave Creek Road, Phoenix, AZ 85024. Contract covers base period December 10, 2000 (or date of contract award, whichever is later), through September 30, 2001, with four 1-year options to extend the term of the contract through September 30, 2005. This acquisition is 100 percent set-aside for small business concerns. North American Industry Classification System (NAICS) Code 561990 and a size standard of $5 million are applicable. All responsible small business sources may submit a Quote. Request for a hard copy of the solicitation must be in writing to Mr. James C. Jackson, Contract Specialist, and accompanied by a check or money order in the amount of Five Dollars ($5.00) (non-refundable), payable to "U.S. Department of Veterans Affairs." Please reference Solicitation Number RFQ 786-12-01 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about November 16, 2000.
