- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-19. RFP 658-18-01 should be referenced on all proposals. The North American Industry Classification System (NAICS) code for this procurement is 62199, and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Medical Center, Salem, Virginia has an emergent need for Two (2) Locum Tenens Physicians Board Certified in Orthopedic Surgery (candidate(s) may be a Board Eligible Physician if within four (4) years of postgraduate residency and/or internship training) to provide surgical, administrative and clinical Orthopedic services at the Veterans Affairs Medical Center, 1970 Roanoke Boulevard, Salem, Virginia 24153. Physician shall provide coverage Monday through Friday, 7:45 a.m. -- 4:30 p.m., excluding Federal Holidays. Physician(s) must have a current Basic Life Saving (BLS) certification. Physicians shall not be required, except in case of emergency, to furnish such services on a National Holiday or during off-duty hours. VAMC Salem, Virginia intends to solicit proposals and award a firm fixed price contract under the authority of Title 38 USC Section 8153. The contract duration for required services is a minimum of six (6) months, but may be extended for an additional six (6) months. Physicians shall perform the following administrative and clinical services at the VAMC, Salem, Virginia, which include the following: (1) Provide Pre-operative and Post-operative Orthopedic Services, including hospital rounds for Orthopedic patients; (2) Conduct weekly outpatient and inpatient clinics on Tuesday, Wednesday, and Friday each week (excluding Federal Holidays)(8:00 a.m. -- 2:00 p.m.); (3) Provide non-operative orthopedic hospital rounds for orthopedic patients; (4) Authentication of Medical Records; (5) Critique Morbidity and Mortality Information and Documentation; (6) Responsible for the dictation of clinic and operation notes, and discharge summaries; (7) Contractor's employees shall provide on-call coverage, which shall be coordinated between the two physicians, and approved by the Acting Chief, Surgical Care Service Line. (8) The Clinic Schedule is as follows: Mondays -- Consult Clinic -- 8:30 a.m. to 3:45 p.m., averages 20 Patients per clinic; Tuesdays, Wednesdays and Fridays -- Consult Clinic -- 8:30 a.m. to 10:00 a.m. averages 4 patients per clinic; Pre-Op Clinic 10:30 to 11:30 a.m., averages 4 patients per clinic; Post-Op Clinic 1:00 p.m. to 3:45 p.m., averages 11 patients per clinic; Thursdays Orthopedic Follow-up/Post Op Clinic 8:30 a.m. to 3:45 p.m., averages 21 patients per clinic; (9) Surgical Schedule is as follows: Mondays and Thursdays are scheduled surgical days (surgical operating room time is shared between the two physicians on these days); Physician(s) shall perform the following operating room surgical schedule: (1) Monday, 8:00 a.m. to 4:30 p.m. (2 to 3 Procedures), and Thursday, 8:00 a.m. to 4:30 p.m. (2 to 3 Procedures); (10) Contractors physician shall be responsible for all major and minor surgical procedures. Offerors shall submit the following with their proposal: pricing schedule; curriculum vitae, medical license (from a State, Territory, or Commonwealth of the United States or the District of Columbia), evidence of board certification, evidence of BLS Certification and DEA certificate of proposed physician(s); and evidence of company's Indemnification and Medical Liability Insurance. Offerors may propose more than one candidate. The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following; Worker's Compensation Insurance, Professional Liability Insurance Income Tax Withholding, Social Security Payments, Health Examinations and, ACLS and BLS Certification. OSHA Regulation Concerning Occupational Exposure to Bloodborne Pathogens: The Contractor shall provide a generic self study training module to its personnel; provide Hepatitis B vaccination series at no cost to its personnel who elect to receive it; maintain and distribute an exposure determination and control plan to its personnel; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident. VA will notify the Contractor of any significant communicable disease exposures as appropriate. The Contractor's occupational health provider shall adhere to current CDC/HICPAC Guidelines for Infection Control in health care personnel (AJIC 1998; 26:289-354) for disease control. The contracting agency shall provide follow-up documentation of employee's clearance to return to the workplace prior to their return. The parties agree that such personnel shall not be considered VAMC employee(s) for any purpose and shall be considered employee(s) of the Contractor. The Contractor shall perform required services in accordance with established principles and ethics of the medical profession established by the AMA, ACEP, JCAHO, the Veterans Health Administration (VHA) regulations, and the policies, procedures, and regulations of the Medical Staff bylaws of the VAMC, Salem, Virginia. Contractor personnel shall provide proof of the following tests within five (5) calendar days after award and prior to the first duty shift. Tests shall be current within the past year. Tuberculosis Testing: All Contractor personnel shall provide proof of a negative reaction to Purified Protein Derivative (PPD) testing. A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually. Rubella Testing: All Contractor personnel shall provide proof of immunization for measles, mumps, rubella or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella immunization must be administered with follow-up documentation to the Contracting Officer's Technical Representative (COTR). Offerors shall provide a completed set of provisions with their offer found at FAR 52.212-3 -- Offeror Representation and Certifications Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4, 52.212-5 apply. The following FAR and VAAR clauses and provisions are added by addendum 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 52.217-8 Option to Extend Services; 52.217-9 Option to Extend the Term of the Contract; 852.237-7 Indemnification and Medical Liability Insurance; 852.270-4 Commercial Advertising; and 852.271-70 Services Provided Beneficiaries. Offerors may find the FAR provisions and clauses on the Internet at www.arnet.gov/far/. Proposals will be accepted immediately, but no later than December 8, 2000. Due to the critical need of these services, award may be made prior to December 8, 2000. The anticipated starting date for these physician services shall be on or around December 18, 2000. An official authorized to bind the offeror's organization must sign all offers.
