The original synopsis/solicitation is revised on this date, 20 Nov 2000 by adding paragraphs 4 and 5 to the Statement of Need, as follows: 4. SITE VISIT. A site visit is mandatory. Please contact TSgt John Callahan at john.callahan@fairchild.af.mil to establish the date and time. All companies interested in providing a quote are required to attend a site visit. Contact TSgt Callahan for access onto the base prior to arriving at the front gate. In addition, a valid driver_s license, proof of current insurance and registration is required to gain access to Fairchild AFB, WA. The purpose of the site visit is to view the existing system, dimensions, and any existing conditions to ensure the customer receives the maximum complete and up-to-date system. Companies that provide a quote and do not attend the site visit will be determined _Non-Responsive_ and will not be eligible for an award. 5. BASIS FOR AWARD. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements and will provide the best value to the Government considering price and past performance. The award is as follows: (!) All responsible offerors whose offer conforms to the solicitation requirements will be ranked by the quoted price from lowest to the highest. (2) The Government reserves the right to award a contract to other than the lowest price offeror if that offeror is not considered to be a low performance risk. (3) If the offeror offering the lowest price offer is considered to be a low performance risk this offeror_s quote represents the best value for this acquisition and award shall be made to the offeror. Offerors shall provide with their proposal a list of at least three of the most recent and relevant past and present contracts in connection with audio visual equipment/communication systems upgrade for Federal Agencies and commercial customers within the last three years. All other terms and conditions remain unchanged.**************** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation F45613-01-T-0001 is issued as a request for quote to purchase, design and install Audio and Video Equipment, 22d Training Squadron, Resistance Training Compound (RTC) at Fairchild AFB, WA per Statement of Work. North American Industry Classification System (NAICS) Code is 334220, size standard $750,000. This acquisition is 100% set-aside for small business. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. The following provisions and clauses can be viewed in full text at http://farsite.hill.af.mil/. The provisions at FAR 52.212-1 Instructions to Offerors- Commercial Items (Oct 2000) and FAR 52.212-3 Offeror Representations and Certifications- Commercial Items (Oct 2000) apply to this acquisition (a completed-signed copy of this provision shall be submitted with any quotation and can be printed from the aforementioned website). The clauses at FAR 52.212-4 Contract Terms and Conditions (May 1999), FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Aug 2000) and DFARS 252.204-7004 (Mar 2000) apply to this acquisition. The following clauses are incorporated under paragraph (b) of FAR 52.212-5: FAR 52.222-21 Prohibition of Segregated Facilities; FAR 52.222-26 Equal Opportunity; FAR 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36 Affirmative Action for Workers with Disabilities; FAR 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; FAR 52.225-1 Buy American Act-Balance of Payments Program-Supplies; FAR 52.225-13 Restrictions on Certain Foreign Purchases; and, FAR 52.232-33 Payment by Electronic Funds Transfer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government based on the factors listed in the Statement of Need.
