Description

Contractor to provide all materials, personnel, and equipment necessary to develop and conduct satisfaction surveys and develop reports on a sample of inpatients and outpatients of the VA Medical Center, Chillicothe, Ohio. Surveys will be taken of approximately 50 inpatients and 50 outpatients per month. Reports on survey will be made no less than quarterly. Contractor shall provide computer access to data on a regular basis. Solicitation provides for a base year and 4 optional one year extensions. Solicitation is issued in accordance with FAR parts 12 and 15. Solicitation available o/a November 27, 2000. Proposals due by 2:00 p.m. local time December 29, 2000.
