Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 (Acquisition of Commercial Items) and FAR Subpart 15 (Contracting by Negotiation). A written solicitation will be issued. Solicitation number 673-61-01 applies and is issued as a request for proposals. This solicitation is a Total Small Business Set-aside. The North American Industry Classification System (NAICS) is 621111 and the business size standard is $5 million. The requirement is for ophthalmology service to be provided at the VA Outpatient Clinic, 2900 Veterans Way, Viera, FL 32940 in accordance with terms and provisions of solicitation. Services shall be performed by board certified ophthalmologist not to exceed 20 hours per week. Period of contract shall be one year for date of award. The government may extend the term of contract three (3) optional years. The above proposal due date should be considered "on or about".
