Description

This is a combined synopsis /solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ No. A72433). The solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-15. The Standard Industrial Classification (SIC) Code for this acquisition is 2813. The Food and Drug Administration (FDA) intends to issue a purchase order for the delivery of Bulk Liquid Nitrogen. The delivery site is Food and Drug Administration, National Institute of Health (NIH), Building 29, Lincoln Drive, Bethesda, Maryland 20892. The contractor must fill the existing 3,000 gallon Liquid Nitrogen Tank with Refrigerated Bulk Liquid Nitrogen as needed based on daily readings from the Government supplied Telemetry Unit. The contractor will monitor Telemetry readings and based upon the readings will generate a delivery. The contractor must provide a printed Delivery Ticket with each delivery. The delivery ticket is required to have the following information: (1) Date and time of delivery, (2) Level of Liquid Nitrogen in inches prior to start of fill up and inches after fill up is completed, (3) Total cost including any additional charges, (4) Total gallons delivered, and (5) Name of Driver/Personnel that delivered for the day. The contractor is required to provide a printed Certificate of Analysis for each bulk delivery (A small mailbox will be located inside of the caged area surrounding the tank for these tickets). The contractor will provide quarterly maintenance on all pipes and fittings and also the tank itself. The contractor must draft a semi-annual report of the average gallons of liquid nitrogen used per month with each delivery date and submit the reports to the Facilities Branch Located in Building 29A, room 1C02. The invoices shall be sent to FDA Facilities, located in building 29A, room 1C02 for approval before going to the FDA Accounting Office for payment. Government furnished property: FDA/CBER will provide Telemetry Unit with the accompanying phone line and high security pad locks/keys for gate. The period of performance shall be January 1, 2001 through December 30, 2001. The award will be made in accordance with FAR Part 12.6 Streamline Procedures for Evaluation and Solicitation for Commercial Items and FAR Part 13.5 Test Program for Certain Commercial Items. The following FAR Clauses shall apply to this solicitation: 52.212-1 Instructions to Offerors _ Commercial, 52.212-4 Contract Terms and Conditions Electronic Funds Transfer, and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. All responsible sources that can provide and meet the above requirement shall submit written quotations by mail or fax 301/827-7039. Evaluation/Award will be based on the technically acceptable quote that offers the best value to the Government Quotes submitted in response to this solicitation shall be submitted by December 12, 2000, 4:00 P.M Eastern Standard time to the above referenced address or fax.
