- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. This procurement is unrestricted. The North American Industry Classification System code is 621111, Offices of Physicians (except Mental Health Specialists. The small business size standard is $7.5 million average annual receipts for the past three years. Request For Proposals (RFP) 515-16-01, Holter Monitor Interpretation services for a period of one year from date of award for the VA Medical Center, Battle Creek, Michigan. Offerors are encouraged to submit quotes based on an estimate of 150 examinations per year to include (1) tracings and summary and (2) Interpretation. Proposed rates should include all costs associated with providing the services off-site. Cardiologists proposed to provide interpretations under this contract shall be board certified in cardiology with a full and unrestricted licensed in any State, Territory, or Commonwealth of the United States or the District of Columbia. The successful contractor shall begin work within 14 days after date of award. The VAMC will provide to the contractor: (a) cassette tapes containing 24 hours of two channel EKG, (b) the patient's standard 12 lead EKG, and (c) the patient's self-completed diary of events for the 24 hour period. Contractor Responsibilities: (1) The contractor shall generate tracings of the 24 hours of two channel EKG; inluding: (a) date and time tracings of High and Low rates, (b) Date and time tracings of dysrhythmias and significant ST abnormalities, (up to 10 of any particular abnormality), (c) Summary of the total number of dysrhythmia and significant ST abnormality types, including the duration of V Tach, PAC'c and pauses, and (d) Tracings shall be delivered to Medical Service (111), VA Medical Center, 5600 W. Dickman Road, Battle Creek, Michigan, 49015, within 7 to 10 calendar days after receipt of the cassettes, (2) If life threatening dysrhythmias occur, the contractor shall notify the VAMC telephone triage immediately. (3) The contractor shall provide a signed/written interpretation, by a board certified cardiologist, of the of the 24 hours of two channel EKG. The interpretation report shall be sent to Medical Service via facsimile, at (616) 660-6011, within 3 working days after receipt of the cassette tape. (4) Provide VAMC a written report including the patient's name, and date the cassette tape was received. This report shall be submitted monthly with the contractor's invoice. All cassette tapes shall be returned to the VAMC. JCAHO Requirements: (1) The Contractor Shall be responsible to ensure that Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. (2) The Contractor is required to maintain records that document the competence/performance level of Contractor employees working under this contract in accordance with JCAHO and other regulatory body requirements. The Contractor shall provide a current copy of the competence assessment checklist and annual performance evaluation to the Contracting Officer's Technical Representative (COTR) for each Contractor employee working under this contract. (3) Where the contract does not require JCAHO accreditation or other regulatory body requirements, the Contractor shall perform the required work in accordance with JCAHO standards. A copy of these standards may be obtained from the Joint Commission on Accreditation of Healthcare Organizations, One Renaissance Blvd., Oakbrook Terrace, IL 60181. The provision at 52.212-1, Instructions to Offerors Commercial (OCT 2000), applies to this acquisition. The provision at 52.212-2, Evaluation -- Commercial Items (JAN 1999), applies to this procurement. Minimum Requirements: (1) Copy of full and unrestricted license to practice medicine in any State, Territory or Commonwealth of the United States or the District of Columbia; and (2) Copy of board certification in Cardiology. Documentation of the minimum requirement stated above shall be submitted with offers. Evaluation criteria included below are, in descending order of importance, (1) Past Performance, (2)Competency, and (3) Price. Past Performance and Competency are significantly more important than Price. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items ALT III (OCT 2000), with its offer.
